Topic: Request for Patient Subject Matter Experts (Patient SMEs)
To enhance the development of our Network activities, the IPRO ESRD Network of New York is seeking at least 10 committed and informed patients to advise in promoting quality of care for their fellow patients.
These patients will serve as Subject Matter Experts (SMEs), to assist the Network in developing programs that will encourage patient and family engagement, by participating in our Patient Learning and Action Network (PFeLAN). Participation would include four conference calls this year, with no travel required. Conference calls will be arranged to accommodate SME schedules. We anticipate that this will be an exciting, productive time that will empower and encourage all patients to be proactive in their care.
A Learning and Action Network (LAN) is an initiative that brings together healthcare professionals, patients and other stakeholders around an evidence-based agenda to achieve rapid, wide-scale improvement. The PFeLAN will be patient-driven with topics chosen by the Patient SMEs participating in the LAN. Patient SMEs will represent the voice of the patient in PFeLAN activities as the Network develops programs to educate and engage the ESRD community in Beneficiary and Family Centered Care.
Please discuss this opportunity with your patients, and encourage them to volunteer to be a SME. Requirements include quarterly conference calls, material development and review and discussion with patients, family members/caregivers and facility staff about PFeLAN activities.
The roles of the Patient SME:
- Attend at least 3 quarterly conference calls per year
- Select patient-centered topics for the QIA and two educational campaigns
- Participate in SME ad hoc meetings as needed in project development
- Review and develop Educational Materials
- Discuss planning for educational events (Webinars, Meetings, Conference Calls)
- Evaluate project interventions
- Share ideas/concerns/thoughts to enhance learning for all
