Going home in your child's folder is a medical information form that must be returned next Tuesday. If your child is absent tonight, May 31st, you can download the travel medical form here. If your child will be absent on Tuesday, June 7th please send the completed form to us electronically.

We also need to double check the information your provided us with when you filled out your Permission to Treat Form at the beginning of the year.  If you have had changes to your medical coverage, contact information, or anything else we need to change please let us know ASAP.
