Hormone Testing: Blood and Saliva

Middle Aged Couple There are several different ways of measuring the hormones in a person's body. Here at Arizona Natural Medicine, we both use blood and saliva tests to evaluate hormone levels. Especially in cases of perimenopause, menopause, irregular or painful menses, painful menses, PMS, low libido, erectile dysfunction or fertility issues, hormone testing can be an effective way to determine how hormones are playing a role in your condition and what can be done about it

.

Both blood and saliva have their advantages and are both viable ways to look at hormones levels. Which test is most appropriate can be determined by your doctor.

 

Most blood and serum tests look only at the level of hormones that are present in a person's tissues. This is known as "bound hormone levels". Saliva testing looks at the "unbound hormone levels", which are the hormone levels that are available to be used by the body's tissues. Saliva testing of hormones gives us a better idea of the levels of hormones that are actually influencing the tissues, rather than just the level of hormones that are present in the tissues.

 

Saliva testing has some advantages over blood testing. Saliva specimen collection does not require a blood draw and the collections are convenient and can be done at home. The ability to collect more than one specimen is another advantage of saliva testing because this can give providers more information than a single collection. This is especially useful with cortisol testing, in the evaluation of adrenal function, that requires 4 samples to be collected in a day.

 

On the other hand, new research of hormones in the blood suggest blood levels represent protein "bound hormones levels", as well as, free fractions ( "unbound hormone levels") to provide a complete picture of the biologically active hormones. Blood testing has been the gold standard when diagnosing or monitoring response to hormone therapies. Traditional bloodwork in capable of testing bound and some (but not all) of free hormone levels.

 

At Arizona Natural Medicine, our physicians, are versed in interpreting both blood and saliva hormone levels. We look for in-depth data when ordering hormone testing.  For example, we test both the free and total levels of testosterone which gives us a better understanding of the biologically active free form in your body that is having an effect on your libido, energy and mood. We also may test additional hormones unlike many physicians, such as all three forms of estrogen (Estrone, Estriol, Estradiol), DHEA sulphate (which is a precursor other sex hormones), along with traditional levels of FSH, LH and Progesterone. Another advantage of blood testing is that it is usually covered by insurance, whereas saliva testing often is not.

 

We have found that both blood and salivary testing of hormones can be very effective in the diagnosis, treatment and managing of hormone related conditions. Which test is most appropriate can be determined by your doctor.

Vitamin B12 - Cobalamin
 
detox foodsVitamin B12, known as cobalamin, was isolated in 1948. It is an important nutrient involved in the synthesis of DNA, myelin sheaths (related to nerves), carbohydrate metabolism and many other biochemical processes in the body. In fact, a deficiency in Vitamin B-12 can lead to a disease called, Pernicious anemia, which can result in neurological damage, anemia, and fatigue. If untreated, it can be fatal. This important nutrient is poorly absorbed without the help of intrinsic factor. Intrinsic factor is secreted by the parietal cells of the stomach and works with your stomach acid and protein enzymes to bind B-12 so is can be absorbed efficiently. This means, that if you have a digestive disorder, routinely use H2 blockers, such as prilosec or pepsid, or if you are under high stress which reduces your normal stomach acid secretion, you could impair you absorption of this nutrient. Other causes of Vitamin B-12 deficiency are pregnancy, celiac disease, colitis, tapeworms, excessive antibiotics, liver disease, and cancer. Vegetarians are at higher risk for a deficiency. Common signs and symptoms of B-12 deficiency include: classic pernicious anemia, peripheral neuropathy, fatigue, depression, confusion, swelling of the tongue, memory loss, and impaired immune response.

 

Although, Vitamin B-12 deficiency can be tested in the blood, many studies have shown clinical improvement is patients with supplementation even if their blood levels were normal.

So how do you prevent a deficiency? Vitamin B-12 is found abundantly in animal products such as beef, salmon, lamb, liver and dairy but can be also found in specific plant sources, such as, brewer's yeast, nori, arame, kombu, spirulina, and chlorella.

 

Vitamin B-12 is used in the treatment of fatigue, anemia (pernicious and due to pregnancy), neuropathy, insomnia, depression, and to treat adrenal decline and adrenal fatigue. It has also been used conjunctively in the treat of asthma, dermatitis, hepatitis, herpes, acne, and psoriasis.

 

 A well rounded whole foods diet with animal protein can offer excellent sources of Vitamin B-12 or supplementation with vitamins can provide additional nutritional source. In addition, Vitamin B-12 can be used as an intra-muscular (IM) injection, which is absorbed readily in to the body, by-passing the gastro-intestinal tract, to maximize absorption and its therapeutic effects. It is also one of the ingredient in the "Myers' cocktail", a nutritional intra-venous (IV) therapy used to treat adrenal fatigue and other disorders. When B-12 is used in an IV or IM injection, the form of cobalamin is essential. Vitamin B-12 comes in three forms for IV and IM use: cyanocobalamin, hydroxycobolamin, and methylcobalamin. Both the hydroxy and methyl forms have a longer effect than the cyano, and the methy form crosses the blood brain barrier while the other forms do not, making it the form used to treat depression and neurological disorders related to brain function.
