A Gift - Express Your Wishes Two
You know what you will want, how you wish to be treated if you get seriously ill.  Have you expressed your wishes?  Does your family, friends, doctor understand what you want?  Have you given them the gift of not having to guess?  

If your answer to these questions is "no" we can help you.  We recently purchased multiple copies of a tool, "Five Wishes", a unique Living Will that shares "your personal, emotional, and spiritual needs as well as your medical wishes."  We want to give you a copy as a gift.  Call 215-219-3521 or email Joanne Luz and tell her which of the "Five Wishes" you are interested in, a hard copy or an online version.  You then can complete, print (with the online version), customize, and then share your wishes. 

Joanne is glad to offer even more help.  She can talk with you and add your information and wishes to the form.  She is also willing to help you in sharing your "Five Wishes" document with your family and friends.   Please accept this invaluable gift.

Five Wishes is the first living will that talks about your personal, emotional and spiritual needs as well as your medical wishes. It lets you choose the person you want to make health care decisions for you if you are not able to make them for yourself.
  • Wish 1: The Person I Want to Make Care Decisions for Me When I Can't - 
    This section is an assignment of a health care agent (also called proxy, surrogate, representative, or health care power of attorney). This person makes medical decisions on your behalf if you are unable to speak for yourself.
  • Wish 2: The Kind of Medical Treatment I Want or Don't Want - This section is a living will - a definition of what life support treatment means to you, and when you would and would not want it.
  • Wish 3: How Comfortable I Want to Be - This section addresses matters of comfort care - what type of pain management you would like, personal grooming and bathing instructions, and whether you would like to know about options for hospice care, among others.
  • Wish 4: How I Want People to Treat Me - This section speaks to personal matters, such as whether you would like to be at home, whether you would like someone to pray at your bedside, among others.
  • Wish 5: What I Want My Loved Ones to Know - This section deals with matters of forgiveness, how you wish to be remembered, and final wishes regarding funeral or memorial plans.

How Five Wishes Can Help YouFour

Five Wishes is written in everyday language and helps people express their wishes in areas that matter most - the personal and spiritual in addition to the medical and legal. It also helps you describe what good care means to you, whether you are seriously ill or not. It allows your caregiver to know exactly what you want. 

Completing Five Wishes is a gift to your family, friends and your doctor because it keeps them out of the difficult position of having to guess what kind of treatment you want or don't want. 

Families also use Five Wishes to help start and guide family conversations about care in times of serious illness.
