Contractor Dies In Fall From Makeshift Platform
An electrical contractor fell off a forklift to his death. When authorities responded to the manufacturing company, they found out a man had fallen about 16 feet from a forklift, suffering serious head injuries. The 47-year-old contractor, had someone use a forklift to place him on top of a wood pallet, creating a platform. The man, and pallet, fell about 16 feet, hitting his head. He was taken to the hospital where he died. OSHA is investigating.
i. LESSONS LEARNED: Lifting people on pallets is "old school," many decades ago it was likely done on a regular basis. It was never safe, but was fairly commonplace if you ask old timers. Putting someone on something, or for that matter in something, and lifting them with a forklift is a bad idea.
ii. SUGGESTED SOLUTIONS: The best solution is to purchase a scissor or boom lift designed for the purpose of lifting people and not things; they are not super costly and are far more productive in conducting elevated work. If you still have a forklift cage in use, ask yourself the following questions: 1) Was it built by a company that makes lifting cages as their business? If not, get rid of it. Homemade cages are a HUGE liability and rarely meet the strict safety requirements.
Have you contacted the manufacturer of every forklift that you use the cage on and obtained an official letter stating they are ok with its use and corresponding data plate change? If not, better toss it in the scrap hopper since OSHA requires this formal approval. Manufacturers rarely provide the go ahead to end users to allow it.
