Coordinating Patient Care

with Netsmart 

 

Care Coordination  

A current shift in healthcare is the drive toward patient-centered care. Healthcare centers are adapting to this shift by reorganizing into care coordination organizations such as patient-centered medical homes, accountable care organizations, and health homes. 

 

The goal of a care coordination organization is to manage the care of its patients being treated by outside organizations and to maintain the information related to that care, while creating better outcomes and reducing costs.  An example of a care coordination organization is shown here.

 

Netsmart CareManager

The HCCN Grant Project Team has evaluated and is considering Netsmart's CareManager as the software solution for care coordination. CareManager is not dependent on any specific EHR. It integrates into existing clinical systems to support population management to:

 

  • Aggregate clinical data to provide a broad picture at the population level
  • Facilitate care coordination across providers
  • Track clinical quality measures and outcomes
  • Manage authorizations and claims across care providers

 

As a member of Health Center Connections, you can utilize CareManager for one year at no cost to your health center. Cost to maintain the software after the grant will be made available to you prior to the implementation of the software.

An accountable care organization is a group of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients. For more information about the accountable care organizations, visit CMS.gov.

 

The KanCare approach to health homes is a team of health professionals which may include a physician, nurse care coordinator, nutritionist, social worker, behavioral health professional (including mental health or substance use disorder providers), and can be free standing, virtual, hospital-based, community mental health centers, etc.   For more information about Kansas health homes, visit KanCare.KS.gov.
