All student athletes must have an updated FHSAA physical (EL2), Consent & Release from Liability Certificate (EL3) and Consent & Release from Liability Certificate for Concussion & Heat-Related Illness (EL3CH), EL3CH Addendum to participate in any school tryout! These forms can be found on the nck12.com website under the Athletics Tab - Forms/Downloads.
NCS Athletic Fee ($150.00) is due once an athlete joins his/her first team of the 2015-16 school year!
