The following letter was mailed to all preschool families earlier this week. If you do not receive this packet of information by Friday, please call Heather Lasseter 727-541-7593 ext. 251. We had originally planned a preschool family meet and greet for Saturday, August 8th, but we will have to postpone the meet and greet until after the start of school due to paving and parking lot construction near the preschool. Dear Preschool Family, Welcome! Your child is enrolled as one of our Mini-Mustangs at Northside Christian Preschool for the 2015-16 school year. As you know, we are looking forward to beginning on August 19, 2015. Before this date, we have some important paperwork to get done. You will find attached the following forms that need to be completed and returned to the registrar's office (located in the high school) on or before the first day of school. These documents are required before your child can attend preschool. - Child's Enrollment Record

- Preschool Discipline Policy
- Food Experience Permission Form
- Flu Brochure
If you have not already provided a copy with your application, we require a copy of your child's birth certificate, social security card, health exam DH3040-yellow form and immunization record DH680-blue form for our files. If you have any questions regarding the forms, please feel free to contact me via phone or email. The attached forms must be filled out completely. We are required by licensing to have all blanks filled out on these forms. If a question does not apply to your child, please note N/A or None where appropriate.
