Dear Friend of Erie,
I want to thank everyone who helped make Erie's 2015 Annual Luncheon the most successful in our history! As a result of your support, we're able to continue advancing our mission to provide compassionate, high quality health care to our current patients as well as thousands still in dire need of care.
In this issue, you'll read about the Annual Luncheon and other recent highlights that are keeping Erie's mission moving forward. For example, Erie HealthReach Waukegan is celebrating its first anniversary!
We're also delighted to welcome fourteen outstanding new primary care doctors into the Class of 2018 of the Northwestern McGaw Family Medicine Residency Programs at Erie Humboldt Park and Erie HealthReach Waukegan. And I'm proud to announce a recent grant that expands access to care for adolescents in our school-based health centers.

Annual Luncheon: Most Successful in Erie History

On Friday, May 15, 400 corporate, philanthropic and community partners gathered at the Four Seasons Chicago for Erie's 2015 Annual Luncheon, Leading Change, Transforming Care. The event raised over $200,000 to support Erie's mission - making it the most successful Annual Luncheon in Erie history.
Christopher G. Kennedy, Chairman of Joseph P. Kennedy Enterprises and Founder and Chairman of Top Box Foods, delivered a keynote address calling on attendees to provide leadership that helps communities in need to access essential care. Honorees (pictured above) included: Julie Ann Janssen, Golden Toothbrush Award; Daniel Johnson, MD, Health Innovator Award; Illinois Chapter, American Academy of Pediatrics, Golden Toothbrush Award; and the Lloyd. A. Fry Foundation, Health Program Champion Award. An event highlight was "My Erie Inspiration," featuring Erie health care providers who shared patient stories.

Erie HealthReach Waukegan On Track To Serve 2,400 Patients In First Year

If steady growth in patient numbers and word-of-mouth referrals indicate success, then Erie HealthReach Waukegan is off to a strong start. In its first ten months of operation, Erie's newest site provided close to 7,000 medical and dental visits to 2,315 patients.
"We're grateful to our partners Northwestern Lake Forest Hospital and Vista Health System," says the site's medical director, Dr. Frances Baxley. "They help us ensure timely continuity of care for patients who need to establish a primary care provider after visiting the emergency room, and they accept our referrals for patients who require specialty, diagnostic and inpatient hospital care." Dr. Baxley also reports that many patients tell providers they heard good things about the health center from neighbors or relatives and decided to check it out for themselves.
Since opening last July, Erie HealthReach Waukegan's services have focused on primary care for adults and children as well as dental care for children and pregnant women. By the end of June, the helth center is projected to serve a total of 2,400 patients. Over the next few years, Erie HealthReach Waukegan will continue to grow. At full capacity, it will serve 10,000 patients.

Welcome To Our New Residents
Erie addresses the severe shortage of primary care physicians by training the next generation through unique and innovative collaborations involving the McGaw Medical Center of Northwestern University, Norwegian American Hospital and Northwestern Lake Forest Hospital. Erie is honored to introduce the 14 remarkable new physicians who were selected to join the Northwestern McGaw Family Medicine Residency Programs' Classes of 2018.
These residents are distinguished by their impressive academic backgrounds and extraordinary volunteer service, and demonstrated passion to practice family medicine in underserved communities. Each residency program promotes quality education and patient care through clinical excellence, research and publication, and leadership opportunities.
Six of the residents will participate in the well-established residency partnership among Northwestern University Feinberg School of Medicine, Norwegian American Hospital and Erie Humboldt Park. Eight will comprise the inaugural class of the Northwestern McGaw Family Medicine Residency at Lake Forest Hospital, receiving outpatient training at Erie HealthReach Waukegan.
Since its launch in 2010, the Humboldt Park-based program has helped provide greater access to care by expanding the primary care workforce in the neediest communities. And it is fulfilling its original promise of developing the primary care workforce of tomorrow: Of the program's 15 graduates to date, 13 currently or plan to practice at Erie and other community health centers.

Erie Family Health Center Awarded $50,000 Grant by the CVS Health Foundation

In April, Erie was honored to receive a $50,000 grant from the CVS Health Foundation in partnership with the School-Based Health Alliance. "This support will help take the health care of our school-based patients to the next level," says Erie President and CEO Dr. Lee Francis. "These young patients will enjoy better electronic access to their Erie providers, their own health information and technology that will assist in addressing their specific health care risks."
The award builds on year one of the partnership, in which Erie received a $100,000 grant to design a patient-centered care model specific to the needs of youth. This year Erie will leverage technology to engage youth in taking charge of their own health and to ensure that they access the care that they need.
"We look forward to continuing to partner with the CVS Health Foundation and the School-Based Health Alliance to expand access to health care for youth in our communities," states Dr. Francis.
