New for the 2014-2015 School Year:
Complete Back-To-School Forms Electronically!
Dear Parents and Guardians,
As the new school year approaches, Beachwood Schools is excited to introduce its new Electronic Forms portal for parents to complete and submit each student's Health History Form, Emergency Medical Authorization and other common back-to-school forms online.
These new Electronic Forms are intended to be a greater convenience for parents and improve records management for our school nurses and other school personnel. Of course, students' information will continue to be maintained with the same high level of confidentiality.
Please follow these instructions to complete back-to-school forms for your children by Friday, Aug. 22:
- Click here to get started at the Beachwood Schools Electronic Forms portal.
- Create your account by clicking on the "SIGN ME UP" box on the right-hand side of the web page.
- You will need your child(ren)'s student ID numbers. (If you do not have this, check your Infinite Campus Parent Portal or ask your children; They use this number to log into email and pay for lunch.)
- Once you've added your child(ren) to your account, select a child from the profile menu at the top to begin completing forms.
- Please reply to this email or contact me for help setting up your account.
Please note:
- This Electronic Forms portal is different than your Infinite Campus Parent Portal account. You'll need to create your Electronic Forms account today to get started.
- The Electronic Forms portal only permits one parent to submit forms for each child. Only custodial parents should register for an account.
- Medication Forms and Action Plans can not be completed online. Instead, you will be prompted to download, print and complete these forms to return to school. Action Plans and Prescription Medication Forms require a physician's signature.
- Electronic Forms are not required for Kindergartners and students who are new to the district, as these forms were already submitted during registration.
