2016-2020 Facility Agreements
2016-2020 Network Facility Agreements have been distributed to all Network facilities and transplant centers. We have utilized a new application called DocuSign to obtain electronic signatures from the facility Medical Director and the Facility Administrator. The Network used CROWNWeb facility personnel information to obtain the appropriate names and e-mail addresses for the individuals who need to sign the agreements.


If you believe you should have received an agreement and did not, or the agreement was not sent to the appropriate individual, please contact Jenna Vonaa at (203) 285-1222.
 
We have also asked for a Network Council Representative as well as a Patient Advisory Committee (PAC) Representative to be nominated by your facility. The Network Council Representative is an elected facility staff member who will serve as a point of contact person and a liaison between the facility and the Network. The PAC Representative is a patient that the facility would like to nominate who could serve as a peer mentor for other patients and represent your facility during PAC conference calls and works closely with the Network Community Outreach Coordinator. CDC Request: Undetermined Cause of Cardiac Arrest During Hemodialysis, 2015 The Connecticut Department of Public Health (CT DPH) has received reports of six patients that experienced sudden cardiovascular collapse leading to cardiac arrest during routine outpatient hemodialysis; three of the six expired. Click here for more information.


Flu Season Begins: Influenza activity is increasing across the country and CDC has received reports of severe influenza illness. Clinicians are reminded to treat suspected influenza in high-risk outpatients, those with progressive disease, and all hospitalized patients with antiviral medications as soon as possible, regardless of negative rapid influenza diagnostic test (RIDT) results and without waiting for RT-PCR testing results. Reviewing Grievance Logs
In 2016, the Network is working on a CMS-driven quality improvement activity (QIA) to review and foster the development of robust and complete grievance logs at the facility level. By  improving the consistency of these logs, facility staff can improve provider/patient relationships and be better prepared for a state survey.

On February 23, 2016 the IPRO ESRD Network Program will be hosting a Webinar to provide a brief overview and review of facility Grievance Logs and provide information about the Grievance project for 2016.

Patient Advisory Committee Representatives
In 2016, every facility is required to have a Patient Advisory Committee (PAC) representative.
 
The PAC consists of dialysis patients, loved ones of dialysis patients, or a transplant recipient. A PAC member must follow their treatment plans, be in good standing with their facility, and demonstrate a positive attitude and concern for quality of care issues. In addition, a PAC member MUST be nominated by a facility staff member. If you have a dialysis patient, family member, or transplant recipient in your facility that you believe would make a  valuable PAC member, please complete a nomination form and return it back to the Network via fax (203) 389-9902 or mail 1952 Whitney Avenue, 2nd Floor, Hamden, CT 06517.

Data Management
CROWNWeb Form Submission
 When submitting CMS-2728 and CMS-2746 forms in CROWNWeb please ensure that your facility are adhering to the CROWNWeb Data Management Guidelines.

  • CMS-2728: within 10 business days of Date Regular Chronic Dialysis Began; no later than 45 days
  • CMS-2746: within 14 days of the date of death

CMS-2744 Annual Facility Survey
  • February 15, 2016- Deadline for generating the 2744
