Community Health Survey

Appalachian District Health Department is currently conducting a county-wide survey to determine health needs and services.  As of this week, there had only been a fraction of the needed/expected results to the survey. This will provide vital feedback to our health department, hospital and community agencies about the needs within our community.  The deadline has been extended until mid-January trying to increase our results, so please take a few minutes to take this short survey.

Answers are completely anonymous and the results can directly impact Alleghany County and services that may be available. Please share with anyone  - advisory boards, friends, family, church groups, clubs (as long as they are over 18 and at least a part-time resident of the county).

The survey may be accessed here.
