Anthem will issue rebates to the small group market only. Impacted employers and subscribers will be notified as required by law.
Nexium 24HR (esomeprazole 20 mg) became available as an over-the-counter (OTC) 14-day treatment for frequent heartburn in May 2014. As a result, Anthem's drug formulary (list of covered drugs) will change effective October 1, 2015. Learn More �

A medication drug exception request will need to be filed by CareFirst members who wish to have their maintenance medication filled for 90 days versus 30 days.

Members, whose drugs are not listed on the Maintenance Drug List, must ask their provider to complete the Maintenance Medication Exception Form.

CareFirst will issue an approval or denial within 72 hours. Authorization for approved drugs will be effective for 5 years. Read the Sales Flash �
