FY2016 ANNUAL STRATEGIC GRANT CYCLE

Cape Cod Healthcare is pleased to release its FY2016 Community Benefits Annual Strategic Grants Cycle Request for Proposals (RFP) on Monday, August 3, 2015.  This RFP is part of an annual competitive grant process focused on addressing the unmet health needs of the residents of Barnstable County.  The CCHC Community Benefits annual competitive RFP grants program is open to all non-profit community organizations with qualifying proposals. NEWS FROM ALZHEIMER'S CAPE COD

Upcoming fundraising auction at the Fine Arts Work Center in Provincetown over Labor Day weekend.  Artists and writers from around the country have come together to make Memory Objects to support free services offered by the Alzheimer's Family Support Center.  All 50 pieces are one of a kind and available for preview online at www.alzheimerscapecod.com.  The gallery opens Friday September 4 at 12:00 Noon, an author reading Friday evening by 7 writers and a fabulous party on Saturday, September 5th at 5 PM.  Bidding will close at 7 PM.  View flyer for more information.

DAVID LEWIS 5K HOSTED BY

CAPE & ISLANDS SUICIDE PREVENTION COALITION REMEMBRANCE CEREMONY   

The next general membership meeting of the Cape & Islands Suicide Prevention Coalition is scheduled for Thursday, September 10th from 11:15 AM - 12:45 PM in Rooms 11/12 of the Barnstable Superior Courthouse, 3195 Main St., Barnstable Village.  Meeting will be preceded by the annual ribbon ceremony remembering those lost to suicide and supporting the friends and families left behind.  The ceremony will take place on the lawn of the courthouse weather permitting or inside if inclement weather. All are welcome to attend.

BARNSTABLE COUNTY HOME CONSORTIUM NOTICE OF PUBLIC HEARING & PUBLIC COMMENT PERIOD   

The Barnstable County HOME Consortium has completed its 2015 Fiscal Year (July 1, 2014- June 30, 2015) utilizing U.S. Department of Housing and Urban Development's (HUD) Federal Fiscal Year 2014 HOME Program funds. The HOME Program was awarded $404,552 to carry out the goals and objectives set forth in the 2010- 2014 Consolidated Plan and Annual Action Plan for this period. A draft Consolidated Annual Performance Evaluation Report (CAPER), outlining the year's accomplishments and challenges is available for public comment.

 

The Consortium will conduct a public hearing on: Thursday, September 17, 2015 - 8:30 AM in the Innovation Room in the Open Cape Building in the Barnstable County Complex, 3195 Main Street, Barnstable, MA 02630.  Read more in English, Spanish or Portuguese.

 

AFFORDABLE HOUSING LOTTERY PROVINCETOWN   

The Community Development Partnership (CDP) is currently accepting applications for an affordable rental housing lottery in Provincetown.  The lottery is for three one-bedroom condos at the Grace Gouveia Building located at 26 Alden Street in Provincetown.  Application deadline is Wednesday, October 14, 2015 at 3:00 PM

 

An Information Session will take place on Saturday, September 12, 2015, 1-2:00 PM and Wednesday, September 23, 2015, 5-6:00 PM at the Provincetown Public Library.

MASSACHUSETTS WORKING ON WELLNESS   

Working on Wellness, Massachusetts, is a new comprehensive statewide worksite wellness initiative to support employers in implementing comprehensive, evidence-based worksite health initiatives. 

 

Working on Wellness provides: a training curriculum that will prepare organizations in developing a Worksite Wellness Action Plan to adopt sustainable policies and practices that promote and protect the health of employees; ongoing technical assistance and capacity building support; a cohort model that provides businesses with a virtual learning community; seed funding to support the launch and sustainability of the program and preparation for eligible businesses to qualify for the Massachusetts Small Business Wellness Tax Credit.

CITIZEN'S LEGISLATIVE SEMINAR   

The Citizen's Legislative Seminar will be held on Tuesday, October 20 and Wednesday, October 21 from 9:30 AM - 4:00 PM.  The seminar is open to the public on a first come, first served basis.  This is an excellent educational seminar for adults of all ages who are interested in learning more about state government and the legislative process.  Topics will include the history and process of the Legislature, the parliamentary role of the Clerk of the Senate and the future of the Legislature.  The CLS culminates with a simulated legislative hearing and Senate session where participants are invited to use what they have learned and participate as "Senators" in the Senate Chamber.

NEW PEER SUPPORT GROUP FOR COMPULSIVE ACQUIRING, SAVING & HOARDING 

IN FALMOUTH   

Falmouth Human Services with the Cape Cod Hoarding Task Force is offering a free support group starting at the end of September for residents of the Upper Cape who have compulsive collecting, saving and hoarding tendencies.  Read more...

DANCE IN THE RAIN WHOLE PERSON APPROACH   

Dance in the Rain Whole Person Approach works to promote healing for people ages 14 years and older who have mental health challenges living on the Cape & Islands.  This peer to peer service is free and no insurance or provider involvement is required. Read more...

HOUSING ASSISTANCE CORPORATION
"BIG FIX"

CAPE COD HOARDING TASK FORCE

TRAINING FOR HOME HEALTH WORKERS   

Jesse Edsell-Vetter, hoarding intervention expert of the Metropolitan Boston Housing Partnership, will join the Cape Cod Hoarding Task Force on October 6, 2015, 9:00 AM - 3:30 PM, Harborview Conference Room, Barnstable County Complex, 3195 Main St. (Rte. 6A) , Barnstable Village to present an advanced hoarding training for home health workers

 

This training will focus on:

  • How to properly assess a hoarding situation
  • How to properly communicate with an individual with hoarding disorder
  • How to help an individual with hoarding disorder sort and discard their belongings.

This event will be tailored to professionals who deal directly with individuals in their homes and address most effective ways to support and assist an individual with hoarding disorder.

House 137 - An Act Establishing a Low Vision Registry and Improved Access to Information in the Commonwealth.  Until a person is declared legally blind, there are no services or help available to them.  This legislation would allow the MA Commission for the Blind to create a registry of significant vision impairment and give those people information about services available to them in their area. Read more...

QUALITY OF LIFE SUMMIT   

Cape Cod Healthcare Quality of Life Management Summit will take place on October 14, 2015 at the Cape Codder Resort.

 

The goal of the Summit is to bring community service providers and members together to start conversations regarding advancing illness and supporting quality of life.  There are so many organizations across the C&I that support high risk populations including elders and chronically ill.  Cape Cod Healthcare hopes this Summit is just the start of bringing together all those who positively impact individuals, caregivers, and families facing advanced or chronic illness and connect the network of support services with providers to improve quality of life and improve transitions throughout an individuals illness.  View brochure  for registration form and additional information.

INTENSIVE BASIC MEDIATION SKILLS TRAINING

SCOPE OF PAIN TRAININGS   

 

Cape Cod Healthcare presents: SCOPE of Pain (Safe and Competent Opioid Prescribing Education) trainings on October 21, 2015, 5:30-9:00 PM at Cape Cod Hospital in Hyannis and October 22, 2015, 5:30-9:00 PM at Falmouth Hospital.  Tuition is free, register now at www.scopeofpain.com.

 

SCOPE of Pain trainings are designed to help safely and competently manage patients with chronic pain, when appropriate, with opioid analgesics.  This training is for physicians, nurse practitioners, physician assistants, dentists, optometrists, podiatrists and pharmacists.  View flyer for more information.

DIALECTICAL BEHAVIOR THERAPY INTENSIVE TRAINING   
Cape Cod Human Services, along with the Linehan Institute/Behavioral Tech from Seattle, WA is co-hosting at 10 day training course in Dialectical Behavior therapy to be held here on the Cape at the Doubletree.  The first weeklong training is October 26-30, 2015 with the follow up training May 2-6, 2016.  Dialectical Behavior Therapy, is the preferred treatment for patients with Borderline Personality Disorder, and provides the best outcomes for those patients with chronic suicidality and self harming behaviors.
 
NATIONAL GRADUATE SCHOOL QUALITY MANAGEMENT PROGRAM 

The National Graduate School Masters of Science in Quality Management Degree Program is designed for individuals who seek a team-driven, results-oriented management program and believe that a management focus in quality and organizational performance is a competitive career advantage. 

Students are typically experienced, working adults from both Corporate and Military environments.

CHILD THERAPY FOR DOMESTIC VIOLENCE SURVIVORS   

The Cape Cod Center for Women (CCCW) is the only emergency domestic violence shelter on Cape Cod and the Islands dedicated to serving all individuals and families fleeing domestic violence.

 

CCCW is launching Child Therapy for Domestic Violence Survivors.  This program will provide free and confidential therapy to children who have experienced domestic violence.  In addition, CCCW will offer free and confidential wrap around counseling to safe parents, siblings and family members.  View flyer.  If you know a child in need please call the CCCW hotline at 508/564-7233 (SAFE).  Services are no cost and no insurance necessary.

BARNSTABLE SENIOR CENTER   

Need Transportation?: The Silver Express transportation program offers dependable service, courteous drivers and short wait times providing transportation to appointments, errands, and even a ride to the Barnstable Senior Center to attend activities.  All vans are handicap accessible and operate Monday - Friday on a first come/first serve basis.  Suggested voluntary contribution of $5 round trip.  Call 508/862-4752 for information. 

 

Adult Supportive Day Program: Designed to provide a comfortable atmosphere for elders to maintain independence, build new friendships and remain active while aging-in-place in the community.  Fees are income-based and transportation may be available.  Program hours: Monday-Friday, 9 AM - 3 PM.  Call 508/862-4765 for more information.

 

Brown Bag for Seniors Program:  The Barnstable Senior Center has a program to help with your food budget.  If  you get or will be getting fuel assistance or live in subsidized housing, or receive food stamps you will be eligible for the Brown Bag for Seniors Program.  The program consists of a free bag of groceries once a month consisting of healthy foods ranging from soup to cereal, meat to vegetables and even an occasional treat!  If you have limited mobility some home deliveries may be available.  Complete the Brown Bag application and mail to Outreach Department, Barnstable Senior Center, 825 Falmouth Rd., Hyannis, MA  02601. STILL AVAILABLE

ACCESS TO HEALTH INSURANCE IS STILL AVAILABLE:  You can apply and enroll in MassHealth at any time during the year. If you are newly eligible for ConnectorCare Plans (subsidized public health insurance*), you can apply and enroll at any time during the year.  You may qualify for a Special Enrollment Period if you have Qualifying Life Events, such as a marriage, having a baby, losing a job, new Massachusetts resident, etc.

  

Community Action Committee of Cape Cod & Islands has certified MA Health Connector Navigators trained to help you!           CAPE COD HOARDING TASK FORCE   

Meetings of the Cape Cod Hoarding Task Force are held the second Thursday of every other month.  Upcoming dates are: October 8 & December 10 at the Dennis Police Department, 90 Bob Crowell Rd., South Dennis.  These meetings are geared towards professionals who work with individuals with hoarding disorder in any capacity.  For more information email ino@hoardingcapecod.org or visit the Cape Cod Hoarding Task Force website.

SAMARITAN SURVIVOR SUPPORT GROUPS

Over 800,000 people worldwide end their lives annually leaving family and close friends who deeply feel the devastation of each loss.  Samaritans on Cape Cod & the Islands has survivor support groups in Falmouth and Hyannis to provide unconditional emotional support, help survivors gain courage and comfort from one another, and deal with a unique grief that is often not understood by others.  For more information contact the Samaritans office at Tel: 508/548-7999

HealthyConnectedCapeCod.org is an easy-to-use online clearinghouse for health and human services resources on Cape Cod.  We have created a single point of entry to connect to an array of online tools .....without having to keep track of every website address!  By visiting one easy-to-remember website address, HealthyConnectedCapeCod.org, users can link to:

  • BCHumanServices.net - Our flagship website highlights community announcements, updates from our regional partners, and Department reports and publications
  • Behavioral Health Portal - Features mental health and substance abuse information, state and federal legislation, self-help guides, and a robust directory of services
  • Health and Wellness Portal - Provides an overview of health data about our region, including County health rankings, and model practices

VETERANS CRISIS HOTLINE

The Veterans Crisis Line (1-800-273-8255, Press 1) is available 24/7/365 to provide confidential counseling and referrals for Veterans and their families.  This is an integrated national outreach effort to increase awareness and use of the Veterans Crisis Line, support and promote broader VA suicide prevention efforts, and promote help-seeking behaviors among Veterans at risk of suicide and other mental health problems. 

MASS 2-1-1 RESOURCE LOCATOR
MASS 2-1-1 is a free, confidential, 24/7 information and referral database and hotline for access to health and human services for consumers and providers.  Multi-lingual translations are also available.  To locate services dial 211 or 1-877-211-MASS on your telephone or go to www.mass211help.org
  
The Massachusetts Emergency Management Agency (MEMA) has designated Mass 2-1-1 as the official point of contact for receiving information during a time of crisis or weather emergency.

To submit materials, please include the following information and forward to kcallahan@barnstablecounty.org :
  • Descriptive Title
  • Brief Description of the Content (the who, what, why, when, where) 
  • Attachments must be submitted in PDF and text format (either DOC or RTF files are acceptable)
  • Weblink or Hyperlink if available 
  • Contact information of the person submitting the information and if appropriate, the contact person for the event (include name, address, phone, TTY, email)
