CAPE COD HEALTHCARE FY2016 ANNUAL STRATEGIC GRANT CYCLE | |
Cape Cod Healthcare is pleased to release its FY2016 Community Benefits Annual Strategic Grants Cycle Request for Proposals (RFP) on Monday, August 3, 2015. This RFP is part of an annual competitive grant process focused on addressing the unmet health needs of the residents of Barnstable County. The CCHC Community Benefits annual competitive RFP grants program is open to all non-profit community organizations with qualifying proposals. Read more...
The FY2016 Annual Strategic Grants program will provide up to $200,000 in grants to support projects ranging from $15,000 to $30,000. Deadline for submission of proposals is September 14, 2015. FALMOUTH Location Legal Consults
WE CAN now serving the Upper Cape! WE CAN is hosting 30-minute informational Legal Consults with a volunteer attorney focusing on Family Law OR Bankruptcy Law on Tuesday August 18, from 10:15 AM - 1:00 PM. Sessions are one to one and registration is required, please call WE CAN (508) 430-8111. These sessions are free to women who can not afford to pay for legal advice.

BRIDGING THE GAP | |
Dance in the Rain is designing a program for families living with mental illness within its core. It seeks to strengthen the family dynamic and bolster the family unit. Come together with families who are living with mental health issues, share your experiences, thoughts and concerns.
Meet peers who have learned to successfully navigate the mental health system and move forward to an empowered life. This is a unique opportunity to strengthen the bonds of family and share with others your experience, strength and hope.

HOPEHEALTH 5TH ANNUAL CELEBRATE SUMMER MEMORIES AUCTION | | The 5th Annual Celebrate Summer Memories Auction will take place on Thursday, August 20, at the Wequassett Resort and Golf Club in Harwich. The auction provides essential funding to provide information, education and support services for those living with Alzheimer's, their caregivers, and the healthcare professionals and community agencies that support them. A DAY OF WELLNESS | | A Day of Wellness will be held at the Cape and Islands Veterans Outreach Center Grace building, 33 Seanest Drive in Mashpee on August 22, 8:00 AM - 3:30 PM. This event will give the veteran community an opportunity to experience various stress-reducing therapies which have been identified as extremely effective in the treatment of anxiety, stress and sleeplessness. BUILDING AND MAINTAINING A COMPREHENSIVE ADULT CITIZENSHIP EDUCATION PROGRAM | | A full-day training event - Building and Maintaining a Comprehensive Adult Citizenship Education Program, hosted by U.S. Citizenship and Immigration Services, will be held on August 27, 2015, 9:00 AM - 4:00 PM at the Thomas P. DEHUMIDIFIER TURN-IN PROGRAM | | Cape Light Compact is offering opportunities to turn-in old dehumidifiers in E. Falmouth on 9/1, Hyannis on 9/18 and West Tisbury on 10/3 and receive a $30 rebate for recycling inefficient units. The unit must be in working order and you must have an active electric account on the Cape or Vineyard. Purchase of a new unit is not required to receive rebate. View flyer for more details. |
HOPEHEALTH OFFERS END-OF-LIFE NURSING EDUCATION CONSORTIUM | |
This professional education program is designed for clinical professionals with a comprehensive overview of the principles involved in achieving quality end-of-life outcomes for veterans and their families experiencing the impact of serious illness.

AFFORDABLE HOUSING LOTTERY PROVINCETOWN | |
The Community Development Partnership (CDP) is currently accepting applications for an affordable rental housing lottery in Provincetown. The lottery is for three one-bedroom condos at the Grace Gouveia Building located at 26 Alden Street in Provincetown. Application deadline is Wednesday, October 14, 2015 at 3:00 PM.
An Information Session will take place on Saturday, September 12, 2015, 1-2:00 PM and Wednesday, September 23, 2015, 5-6:00 PM at the Provincetown Public Library.

SWIM FOR LIFE AND PADDLER FLOTILLA | |
The 28th Annual Swim for Life and Paddler Flotilla to benefit the AIDS Support Group of Cape Cod will take place on Saturday, September 12th in Provincetown. The swim is a very important and moving community tradition with participants swimming from Long Point across the harbor to the Boatslip.

FALMOUTH'S 3rd ANNUAL EMERGENCY PREPAREDNESS & FIRE PREVENTION FAIR | |
Falmouth Emergency Management Director Mark Sullivan and the Falmouth Local Emergency Planning Committee (LEPC) are hosting a Community Emergency Preparedness Fair on Saturday, September 19, 2015, from 10 AM to 2 PM at Gus Canty Recreation Center, 790 Main St., Falmouth.
During the last several winter storms many of our community members who came to the shelter or sheltered in place were unprepared. Many spend days without heat or electricity, without a means of keeping warm or cooking for themselves or their families during several very cold days. We want to do all we can to educate and increase preparedness for next time (because we KNOW there WILL be a next time).
Residents can learn first-hand how to Prepare an Emergency Supply Kit, Prepare Pets for Sheltering, Prepare Elder and Disabled persons for Sheltering, become a CERT Volunteer, learn about New England's Hurricane History, How to Grow your own Sustainable Food Source, and How to Financially Prepare for a Disaster. PARKINSON SUPPORT NETWORK OF CAPE COD FALL FLING DINNER CELEBRATION | |
The Parkinson Support Network of Cape Cod will hold its 7th Annual Fall Fling at the Cape Codder Resort and Spa on September 27th from 4 - 7:00 PM. The Fall Fling is an evening of camaraderie, dining and entertainment for those living with Parkinson's disease, their families, friends and supporters. The event provides essential funding for the Parkinson Support Network of Cape Cod and helps provide educational series through the year, a caregiver symposium, and support for individuals with Parkinson's and their families. QUALITY OF LIFE SUMMIT | |
Cape Cod Healthcare Qualify of Life Management Summit will take place on October 14, 2015 at the Cape Codder Resort.
The goal of the Summit is to bring the community service provide3rs and members together to start conversations regarding advancing illness and supporting quality of life. There are so many organizations across the C&I that support high risk populations including elders and chronically ill. Cape Cod Healthcare hopes this Summit is just the start of bringing together all those who positively impact individuals, caregivers, and families facing advanced or chronic illness and connect the network of support services with providers to improve quality of life and improve transitions throughout an individuals illness. CAPE & ISLANDS SUICIDE PREVENTION COALITION REMEMBRANCE CEREMONY | | The next general membership meeting of the Cape & Islands Suicide Prevention Coalition is scheduled for Thursday, September 10th from 11:15 AM - 12:45 PM in Rooms 11/12 of the Barnstable Superior Courthouse, 3195 Main St., Barnstable Village. Meeting will be preceded by the annual ribbon ceremony remembering those lost to suicide and supporting the friends and families left behind. The ceremony will take place on the lawn of the courthouse weather permitting or inside if inclement weather. All are welcome to attend. |
BARNSTABLE COUNTY HOME CONSORTIUM NOTICE OF PUBLIC HEARING & PUBLIC COMMENT PERIOD | |
The Barnstable County HOME Consortium has completed its 2015 Fiscal Year (July 1, 2014- June 30, 2015) utilizing U.S. Department of Housing and Urban Development's (HUD) Federal Fiscal Year 2014 HOME Program funds. The HOME Program was awarded $404,552 to carry out the goals and objectives set forth in the 2010- 2014 Consolidated Plan and Annual Action Plan for this period. A draft Consolidated Annual Performance Evaluation Report (CAPER), outlining the year's accomplishments and challenges is available for public comment.

MENTAL HEALTH CONCERN NOTIFICATION INITIATIVE | |
July 14, 2015: the creation of a new initiative for processing criminal cases which may involve a person living with mental illness announced.
The Mental Health Concern Notification System enables law enforcement personnel to take certain information contained in the regular police report and put it on a special form designed for this system. The form is simply for the purpose of notification to the DA's office for an expedited review of the case and possibility of alternative processing. Law enforcement personnel will not be making a determination of any kind regarding the person's mental health but simply calling attention to a concern. Read more...

BARNSTABLE ADULT SUPPORTIVE DAY PROGRAM IS OPEN | |
The Barnstable Adult Supportive Day Program, located in the Barnstable Senior Center, 825 Falmouth Rd., Hyannis, has been supporting elders and caregivers in our community for 17 years. The program is a community-based, professionally operated program which has been an integral part of the Town of Barnstable and a trusted resource for local families. The Adult Supportive Day Program provides a safe, comfortable and welcoming environment to elders/seniors who cannot, or wish not to stay home alone during the day.
If you are a caregiver in need of respite, or a senior who many benefit from the program please call 508/862-4765 to schedule an appointment for a tour and to discuss how the program may help meet your needs.

ACCESS TO HEALTH INSURANCE IS STILL AVAILABLE: You can apply and enroll in MassHealth at any time during the year. If you are newly eligible for ConnectorCare Plans (subsidized public health insurance*), you can apply and enroll at any time during the year. You may qualify for a Special Enrollment Period if you have Qualifying Life Events, such as a marriage, having a baby, losing a job, new Massachusetts resident, etc.

CAPE COD HOARDING TASK FORCE | |
Meetings of the Cape Cod Hoarding Task Force are held the second Thursday of every other month. Upcoming dates are: October 8 & December 10 at the Dennis Police Department, 90 Bob Crowell Rd., South Dennis. These meetings are geared towards professionals who work with individuals with hoarding disorder in any capacity. This is an integrated national outreach effort to increase awareness and use of the Veterans Crisis Line, support and promote broader VA suicide prevention efforts, and promote help-seeking behaviors among Veterans at risk of suicide and other mental health problems. |
