Welcome to the information E-Newsletter from Barnstable County Department of Human Services.  This newsletter is designed to provide timely information on health and human service topics of interest for people living and working in Barnstable County.  For additional information visit the Department website at bchumanservices.net
MONEY FOLLOWS THE PERSON:  A COMMUNITY SUPPORT SERVICE PROGRAM IN MASSACHUSETTS

"Money Follows the Person" (MFP) is a five year Demonstration grant from the Centers of Medicare and Medicaid Services (CMS) that helps states rebalance their long-term care systems and thereby transition people with Medicaid from long term care facilities to their own homes and apartments in the community.  MFP represents a new direction in supporting people with long-term care needs by providing an array of community-based support services, funding for rental security deposits and resources for other basis home furnishings.  These combined supports assist an individual to transition to the community and live with increased independence.

COMMUNITY HEALTH NETWORK AREA (CHNA 27) SEEKS ADMINISTRATIVE ASSISTANT

The Cape & Islands Community Health Network Area 27 (CHNA 27) is seeking an Administrative Assistant to provide organizational and administrative support to the Chair, Steering Committee and general membership.  This is a contract position for approximately 15 hours per month, including two required monthly meetings; one in Barnstable on the second Thursday of the month (9:00-11:00 A.M.) and the other in Yarmouthport on the last Tuesday of the month (3:00-4:30 P.M.).

 

Must be detail oriented and Microsoft Office proficient.  Responsibilities include but are not limited to: meeting preparation, note taking, record keeping and retention. View job description.

 

The purpose of CHNA is to learn about agencies and services and other members of the continuum of care; to support and collaborate with members around issues and problems and to acquire new information and skills.

CAPE COD HEALTHCARE'S FY2015 STRATEGIC ANNUAL GRANT CYCLE
Cape Cod Healthcare will release their FY 2015 Community Benefits Annual Strategic Grants Cycle Request for Proposals (RFP) on Friday, August 8, 2014. The RFP and required proposal attachment files will be available on Cape Cod Healthcare's website at www.capecodhealth.org/communitybenefits

HOPE HEALTH CELEBRATE SUMMER MEMORIES AUCTION

The "Celebrate Summer Memories Auction" event to benefit Hope Health dementia and Alzheimer's programs and services will take place on Thursday August 21st at the Wequassett Resort and Golf Club in Harwich.  This elegant evening includes hors d'oeuvres, live music from the Mark Greel Band, and a life and silent auction.

YMCA DIABETES PREVENTION PROGRAM

NOW ENROLLING!!!

Participants gather in a relaxed classroom setting and work together in small groups to learn how to incorporate healthier eating and moderate physical activity into their daily lives. This class is led by a trained Lifestyle Coach for a 1-year program.  The year long lifestyle change is divided 16 weekly sessions followed by 8 monthly sessions.  The program goals are to reduce body weight by 7% and to increase physical activity to 150 minutes per week.  To learn more about this program please contact Patience AT 508-362-6500 x 136 or pcabrera@ymcacapecod.orgView flyer for more details.

IGNITING THE CREATIVE MIND: EXPRESSIVE ARTS WORKSHOPS

When creativity is fostered in those with mental illness amazing and powerful things can happen.  As a person begins to explore their creativity through the arts, the mind begins to slowly come to life in areas that may have been dormant because of illness or past experiences.

HEALTHY AGING OPPORTUNITIES

My Life, My Health Cape Cod is working together to bring evidence-based healthier aging opportunities to Cape Cod.  FREE six week workshops will be held at:

Chronic Disease Self-Management is for anyone living with an on-going medical condition like arthritis, asthma, cancer, chronic back pain, chronic fatigue syndrome, COPD, diabetes, fibromyalgia, heart disease, high blood pressure, Parkinson's disease, stroke and more.

BACK PACK DRIVE IS HERE AGAIN! 

Help a child in need to start off the school year right!  The Independence House Backpack Drive is back for a third year and your help is needed.  Independence House is looking to support the clients of their Child Witness to Violence and Teen Programs as well as children of adult clients in need of much needed supplies for school.  Independence House is accepting donations until August 17th.  Call 508/771-6507 for more information.  Drop off location:  160 Bassett Lane Hyannis between 8:00 AM - 5:00 PM, Monday - Friday.

LITTLE FIX PROGRAM

The Little Fix Program Mission: Coordinated by The Provincetown Housing Office and funded with Provincetown Community Preservation Act Funds. The Little Fix will send teams of volunteers to assist homeowners & renters with clean-up, landscaping, and small home repair projects targeting seniors, disabled, and veterans. The goal is to provide small home and landscaping improvements, largely through a volunteer base, to enhance the safety, comfort, and quality of life for seniors, veterans, and the disabled. 

 

The residents will be selected based on income, repair needs, and our ability to meet the request. We anticipate our FIRST Little Fix volunteer day will be in September 2014. Volunteer applications may be considered for that date and/or a future Little Fix program day. A variety of volunteers will be needed to perform small landscaping, and home improvement projects. Volunteer assistance will also be needed for coordination of teams and refreshments.  View program informationView volunteer information.

CAPE MEDIATION'S FALL 2014 TRAINING

Intensive Mediation Skills Training: October 20, 21, 27 & 28, 2014

 

Cost $550.00.  $495 if payment received by September 20th.  A limited number of partial scholarships available thanks to the Peter Regan Scholarship Fund. 

 

This program has been approved for 30 Social Work Continuing Education hours for relicensure.  Also certified by MMCEP and is approved as a Continuing Education course of activity for 30 hours for Licensed Mental Health Counselors.

LAUNCH OF WINTER FARMERS' MARKET ON LOWER CAPE

The Community Development Partnership is excited to announce the launch of the first ever Winter Farmers' Market on the Lower Cape! This announcement comes during National and MA Farmers' Market Week, August 3-9. View Governor Patrick's MA Farmers' Market Week proclamation.  We are pleased to be working with local growers and producers to expand their season, and to offer residents local produce opportunities year round. 

 

The Orleans Winter Farmers' Market will take place at the Nauset Regional Middle School on the first and third Saturdays of the month from 10:00 AM to 12 Noon, beginning on Saturday December 6th through April 18, 2015.  A project of the CDP, the Market is offered with support from the Orleans Winter Farmers' Market Advisory Committee and made possible by financial support fromt he USDA Rural Development Program and the Cape Cod Economic Development Council.  Read more...

CAPE & ISLANDS SUICIDE BULLETIN JULY 2014 FACT SHEET

The Department of Human Services recently released a summary of suicide data for the Cape and Islands through 2011, the most recent date that data is available.  This fact sheet summarizes data on suicide deaths specific to residents of Cape Cod (Barnstable County), Martha's Vineyard (Dukes County) and Nantucket during the period 2000 to 2011.  View the entire report.

TIPS AND RESOURCES FOR QUITTING SMOKING DURING PREGNANCY

In Barnstable County, an alarming number of mothers smoke while they are pregnant.  According to recent data, Barnstable's rate of smoking during pregnancy is 79% higher than the statewide average.  According to the recent U.S. Surgeon General's report, smoking during pregnancy can cause a variety of complications and birth defects.  Read more ...

 

www.Makesmokinghistory.org is a great place to learn new information on how to quit and find resources available to help.  The Massachusetts Smokers' Helpline is 1-800-QUIT-Now (1-800-784-8669).

HOPE HEALTH RECEIVES GRANT TO EXPAND CARES PROGRAM

Hope Dementia & Alzheimer's Services announced the expansion of their CARES program, thanks to a generous grant from the Tufts Health Plan Foundation. The unique CARES support group model provides an opportunity for people with memory impairment and their care partners to participate in two separate support groups-held simultaneously. One group provides information, guidance and respite to caregivers while the other group provides conversation and socialization for those with memory impairment.

 

Currently CARES groups are held monthly in 13 locations in eastern Massachusetts, including Bourne, Carver, Chatham, Dennis, Eastham, Harwich, Mashpee and Wellfleet.

  

Hope Dementia & Alzheimer's Services provides a wide range of services for those living with Alzheimer's or a related dementia, and for their caregivers, healthcare professionals and community agencies. The funds raised from donors such as the Tufts Health Plan Foundation are used to provide free services to the community

Massachusetts Substance Abuse Helpline Connects People with Services:  For tens of thousands of Massachusetts resident struggling with alcohol and other drug related issues, 1-800-327-5050 is an important number.   Read more ... 

BARNSTABLE COUNTY DEPARTMENT OF HUMAN SERVICES ON-LINE RESOURCE DIRECTORIES

The Barnstable County Department of Human Services website find.bchumanservices.net features on-line directories of health and human service resources. The Directories are a compilation of local resources organized around areas of critical need such as food, housing,  energy/fuel assistance, and services for seniors and people with disabilities. Find.bchumanservices.net is a valuable resource now available to Barnstable County residents and human service organizations. This new on-line tool is an effective and efficient way to make local information available on a 'simple to search' website. For more information view press release in its entirety.

The Massachusetts Emergency Management Agency (MEMA) has designated Mass 2-1-1 as the official point of contact for receiving information during a time of crisis or weather emergency.

VETERANS CRISIS HOTLINE

The Veterans Crisis Line (1-800-273-8255, Press 1) is available 24/7/365 to provide confidential counseling and referrals for Veterans and their families.  This is an integrated national outreach effort to increase awareness and use of the Veterans Crisis Line, support and promote broader VA suicide prevention efforts, and promote help-seeking behaviors among Veterans at risk of suicide and other mental health problems.  For more information about the Veterans Crisis Line please click here.

SAMARITAN SURVIVOR SUPPORT GROUPS

Over 800,000 people worldwide end their lives annually.  Seven to ten people -- parents, children, siblings, aunts, uncles, grandparents, grandchildren and close friends -- deeply feel the devastation of each loss.  That means that approximately 5 million "survivors" are left behind every year.  

  
Samaritans has sites on the Cape & Islands for their survivor support groups including Falmouth, Hyannis and Nantucket.  These groups honor and respect the needs of the survivors, provide unconditional emotional support, help survivors gain courage and comfort from one another, and deal with a unique grief that is often not understood by others.
