Navigator Pilot Project Grant Award    

 

The Department of Human Services is pleased to announce that it has awarded a $ 90,000.00 grant to the Visiting Nurse Association of Cape Cod for an 18 month Navigator Pilot Project.  The purpose of the project is to improve access to information and the coordination and delivery of home based long term services and supports.  The project will assist residents of Cape Cod aged 18 and over who have chronic health conditions and disabilities, regardless of income.

 

The Navigator Pilot Project will assist individuals and their families navigate the long term services and supports system to access needed services.  The Navigator is intended to enhance access to existing services and will be offered to individuals who require intensive short term advocacy and assistance to 'navigate' multiple service systems so they have the services and supports to remain as independent and healthy as possible.  The Navigator Pilot does not duplicate existing programs and services.  

 

The VNA has a long history of providing community based services and were chosen through a competitive RFP process.  Over the next several months, the VNA will begin to conduct outreach to organizations and groups on how to access the Navigator Project.  Funding for the grant is made possible by Barnstable County, CHNA 27, and Cape Cod Health Care.

TRAINING: ACCESSING AFFORDABLE HOUSING

On Tuesday, May 21 at 9:30 a.m. come learn about the resources provided by our 13 local housing authorities to help house low income, elderly, disabled and family households. We'll also provide a brief introduction to the new on-line Housing Directory on the Barnstable County Human Services website. This training will be held at the Harborview Conference Room at the Barnstable County Complex, 3195 Main St., Barnstable.

 

This is Part 1 of a free training series on Accessing Affordable Housing Resources sponsored by the Client Coordination Council of the C&I Regional Network to Address Homelessness.  Part 2 is scheduled for Tuesday, June 18 and will cover accessing 1) private affordable housing and 2) permanent supportive housing for the homeless funded by HUD's McKinney-Vento  program.

SUPPORT FOR PEOPLE WITH INTELLECTUAL & DEVELOPMENTAL DISABILITIES

The 18 Chapters of The Arc in Massachusetts along with the more than 700 Chapters nationwide are the driving force behind human rights advancements for people with intellectual and developmental disabilities.  A cornerstone of our mission is advancing the opportunities for those we serve to determine their own path in life, to choose where and how they live and work.  On May 18th, the Chapters of The Arc in Massachusetts are coming together for the third annual All Aboard The Arc event.  We are raising money to sponsor participation by our members in this event, which honors their voice and achievements while providing the resources to continue to advance the human rights of people with I/DD.  This year's event will be at the Hynes Convention Center  in Boston and will feature self-advocates, Flame (a band of musicians with I/DD), Gabbie Rae (a pop singer who has waged an anti-bullying campaign) and the combined enthusiasm of those we serve.

 

It has been a difficult spring in Boston and Massachusetts with incidents demonstrating the stark contrast of human nature at its worst and best.  Arc is asking for your support in sponsoring riders for this year's All Aboard The Arc event as another step toward our better natures.  To make a donation, please go to our website www.arcofcapecod.org.  Arc truly appreciates your support for people with intellectual and developmental disabilities.

SPAULDING REHABILITATION HOSPITAL ADAPTIVE SPORTS FAIR
SPAULDING ADAPTIVE SPORTS FAIR PUTS RECREATION WITHIN REACH FOR PHYSICALLY CHALLENGED

 

Joining in sports and recreational activities is often difficult for persons with physical and cognitive challenges, but those barriers can be overcome with the right equipment and training.  Spaulding Rehabilitation Hospital Cape Cod will hold its fourth annual Adaptive Sports Fair on Saturday, May 18, from 11:00 a.m. to 3:00 p.m. at the hospital in Sandwich.  The event is an opportunity for those with disabling conditions to learn about adaptive sports, including cycling, kayaking, golfing and camping.  Spaulding is holding this free event in collaboration with CAPEable Adventures, Northeast Passage, and New England Disabled Sports.  Spaulding Cape Cod is located at 311 Service Rd., E. Sandwich. 

 

The fair is intended to help those living with limitations try new activities or reconnect with recreation they enjoyed previously.  It is for persons of all ages and abilities who may be challenged with balance, dexterity, pain or mobility.  Past participants have included persons with stroke, spinal cord injury, traumatic brain injury, amputation, arthritis, MS, and cerebral palsy, as well as those recovering from surgery, back pain and injuries.   "It's an opportunity to introduce non-athletes to activities they may have thought were beyond their capabilities," says Mary Patstone, Director of Spaulding Adaptive Sports Centers (SASC). FAMILY PANTRY IN HARWICH SEEKS WAREHOUSE SUPERVISOR

The Family Pantry of Cape Cod in Harwich seeks to hire a part-time year-round Warehouse Supervisor, 15-20 hours per week. Responsibilities include managing warehouse volunteers, food deliveries and storage, recycling center, food acquisition and building maintenance.  Must be a team player and able to communicate with groups of volunteers via email.  Salary is commensurate with experience. The Statewide Homeless/HIV Integration Project (SH/HIP) partners with homeless shelters to promote best practices in addressing the issue of HIV/AIDS among people experiencing homelessness.  HIV integration in homeless service settings promotes HIV discussions, education, support, and referrals with people who are experiencing homelessness.  Services include needs assessment, staff training, curriculum development, and resource development and dissemination. In exploring HIV integration strategies, we build on the strengths and culture of each of our partners.

HEALTH & DISABILITY PROGRAM: HEALTH NEEDS SURVEY FOR PEOPLE WITH DISABILITIES IN MASSACHUSETTS

The Health and Disability Program, part of Office of Health Equity at the MA Department of Public Health (DPH) is conducting a health needs survey for people with disabilities in Massachusetts. The Office of Health Equity promotes the health and well being of minority populations, including people with disabilities throughout the Commonwealth. Results from the survey will be used to determine how best to address the current public health needs of the disability community. To that end, first, please take a few moments to complete the health needs survey yourself at http://www.surveygizmo.com/s3/1219419/Health-Needs-Assessment-2013B.  Survey link active until May 31, 2013.  This is a voluntary and anonymous survey.  Please forward link to friends and colleagues in the disability community if applicable.

MASS 2-1-1 NEW INFORMATION AND REFERRAL RESOURCE FOR FAMILIES & CHILDREN REQUIRING ASSISTANCE

Mass 2-1-1 is an information and referral service for children, youth and families offered by the Executive Office of Health and Human Services. EOHHS has contracted with MASS 2-1-1 Inc. to provide enhanced information and referral services to families and children who are dealing with challenging issues.

 

This program supports all families, including families who are involved with Chapter 240 of the Acts of 2012, also known as "Families and Children Engaged in Services" legislation. The legislation, which became effective in November of 2012, provides services for children who are runaways or truants, or children who are having serious problems at home and in school; the law also helps children who are victims of commercial sexual exploitation.

 

Families can dial 211 from any phone and be connected to a trained information and referral specialist, anytime of the day or night.

The Barnstable County Department of Human Services is pleased to announce a new website find.bchumanservices.net featuring on-line directories of health and human service resources. The Directories are a compilation of local resources organized around areas of critical need such as food, housing, mental health services, energy/fuel assistance, and services for seniors and people with disabilities. Find.bchumanservices.net is a valuable resource now available to Barnstable County residents and human service organizations. This new on-line tool is an effective and efficient way to make local information available on a 'simple to search' website. For more information view press release in its entirety.

MASS 2-1-1 is a free, confidential, 24/7 information and referral database and hotline for access to health and human services for consumers and providers.  Multi-lingual translations are also available.  The Massachusetts Emergency Management Agency (MEMA) has designated it as the official point of contact for receiving emergency information during a time of crisis.

VETERANS CRISIS HOTLINE

The Veterans Crisis Line (1-800-273-8255, Press 1) is available 24/7/365 to provide confidential counseling and referrals for Veterans and their families.  This is an integrated national outreach effort to increase awareness and use of the Veterans Crisis Line, support and promote broader VA suicide prevention efforts, and promote help-seeking behaviors among Veterans at risk of suicide and other mental health problems.  For more information about the Veterans Crisis Line please click here.

SAMARITAN SURVIVOR SUPPORT GROUPS
Over 800,000 people worldwide end their lives annually.  Seven to ten people -- parents, children, siblings, aunts, uncles, grandparents, grandchildren and close friends -- deeply feel the devastation of each loss.  
That means that approximately 5 million "survivors" are left behind every year.
Samaritans has expanded the number of sites on the Cape & Islands for their survivor support groups including Falmouth, Hyannis and Nantucket.  These groups honor and respect the needs of the survivors, provide unconditional emotional support, help survivors gain courage and comfort from one another, and deal with a unique grief that is often not understood by others.
