Free resource, Find MI Care, available to health care consumers and providers; continues to expand across Michigan
|
More than a year following the launch of the breakthrough Find MI Care website and mobile app, powered by the Greater Detroit Area Health Council [GDAHC], we are expanding the reach into more counties. In addition to the seven-county southeast Michigan region, we have begun adding clinics from Ingham County (Lansing).
Did you know that Find MI Care is a free resource available to consumers, providers -- including clinics that want to be listed -- employers, and health plans?
Find MI Care enables consumers to locate free or low-cost health care services, such as physicians and dentists, near their home or work. The website and mobile app for Apple and Android devices are frequently updated.
- Search for free clinics and Federally-Qualified Health Centers (FQHCs) in the seven-county southeast Michigan region (expanding to additional counties).
- Find a clinic within a specific geographic radius, including by cross streets, city, zip code and region.
- Print a list of clinics that meet search criteria or print detailed information about specific clinic(s).
- Access information on community resources, including hotlines and links to social services.
- View clinic search results which include:
- Clinic name and photo;
- Address and map (the Google map is printable);
- Phone number(s);
- List of services offered;
- Hours of operation; and
- Clinic-specific information, such as what to bring to an appointment, when to call, languages spoken, etc.
- Visit FindMICare.org and/or download the free app to Apple or Android mobile device(s).
- Individuals may search for clinics in their area, specifically by cross streets, city, zip code or region.
- Hospitals, physicians and clinic staff may print clinic profiles near the patient's home during the discharge process.
- Community groups may use the website and mobile app to assist in connecting people to care.
- All may use Find MI Care to access links to other helpful resources.
- Connect people to free and low-cost care.
- Reduce Emergency Department (ED) visits and inpatient re-admissions due to lack of appropriate follow-up post-discharge.
- Lessen ED visits by providing patients with a care alternative.
- Enhance patient experience of care by improving care transitions and reducing inappropriate care.
- Help the newly-insured get access to care.
What resources are available from GDAHC?
- Staff are available to provide on-site group instruction on the use, benefits and capabilities of Find MI Care.
- Brochures, flyers and posters may be requested from GDAHC.
- Co-branding the Find MI Care materials for your facility can be considered.
