Timing the Discussion of Anti-obesity Medications during Obesity Treatment
Rebello, C. J., et al. Obesity, 2016
The time at which anti-obesity medications should be added to an obesity intervention has not been agreed upon in treatment guidelines. Many physicians do not even discuss weight loss unless the patient brings it up, let alone discussing anti-obesity medications. Patients with obesity who enroll in a lifestyle program often stop treatment before there is any discussion of a pharmacologic intervention. To prevent non-responders from dropping out of weight-loss programs, it is crucial that experts identify the optimal time to treat patients more aggressively. The authors suggest that if a patient does not achieve 5% weight loss by 12 weeks of a lifestyle intervention program, anti-obesity medications may be considered. If the patient loses greater than 5% at 12 weeks, physicians may consider deferring anti-obesity medications until problems with maintenance develop. A discussion about weight regain and pharmacotherapy may be appropriate while weight loss is still happening to ensure the patient is aware of the pharmacologic option when needed, given the physiologic forces predisposing to weight regain. The authors suggest that weight loss may be better maintained and even improved with anti-obesity medications. Specific criteria are needed to assist physicians on timing the incorporation of anti-obesity medications in patients with obesity. View article
Effects of Offering Look-alike Products as Smart Snacks in Schools
Harris, J. L., et al. Childhood Obesity, 2016
Smart Snack packaging is intended to be a more nutritious version of a similar-looking but less nutritious snack sold outside of schools (look-alike products). A study was conducted to determine if the Smart Snacks confuse consumers about their nutritional quality. Results showed that students and parents rated look-alike and store versions similarly in taste, healthfulness, and purchase intent, while considering repackaged Smart Snacks as healthier but less tasty. They also rated schools that offered look-alike Smart Snacks and store versions as less concerned about students' health and well-being. The authors concluded that having Smart Snacks sold in schools may improve the nutritional quality of the snacks offered, but this practice likely benefits the brands more than the children's overall diet and undermines each school's ability to teach good nutrition. The Relationship between Early Weight Loss and Weight Loss at One Year with Naltrexone ER/Bupropion ER Combination Therapy
Fujioka, K., et al. International Journal of Obesity, 2016.
Patients who do not respond to pharmacotherapy by achieving >5% or clinically meaningful weight loss should discontinue that particular therapy. A study was conducted to test the relationship between early weight loss and long-term weight loss in patients who achieved 5% weight loss by week 16 of the phase-3 naltrexone/bupropion 32/360 mg clinical trial (NB). NB participants (n=873) who completed one year of treatment and had weight loss of at least 5% at week 16 were shown to have weight loss of 11.7% at week 56. Eighty-five percent of these participants had weight loss of >5% at week 56. By looking at week 16 weight loss, 80% of the participants who would and would not achieve >5% weight loss at week 56 were correctly identified. This study suggests that after 16 weeks of treatment, physicians may be able to identify patients who likely will and will not succeed in achieving clinically meaningful weight loss after one year of treatment.
