Senate and House Obesity Leaders Introduce National Obesity Care Week Resolution
This month, Senators Tom Carper (D-DE) and Shelley Moore Capito (R-WV) and Representatives Eric Paulsen (R-MN) and Ron Kind (D-WI) introduced Senate Res. 536 and House Res. 840, respectively, which would proclaim the week of Oct. 30-Nov. 5, 2016, National Obesity Care Week. The goal of National Obesity Care Week is to encourage all people in the United States to create a foundation for open communication to break barriers of misunderstanding and stigma about obesity and to improve the lives of all individuals affected by obesity and their families.

The Importance of the Edmonton Obesity Staging System in Predicting Postoperative Outcome and 30-Day Mortality after Metabolic Surgery

The Edmonton Obesity Staging System (EOSS) was applied in the selection of patients for bariatric surgery to determine if it could predict postoperative outcomes and 30-day mortality after surgery. Results showed that of the 534 patients included, the total postoperative complication rate was 9%. Seventy-one percent of patients were in EOSS stage 2, followed by stages 3 (12.6%), 1 (11.6%), and 0 (5.1%). The postoperative complication rates after laparoscopic Roux-en-Y gastric bypass, laparoscopic sleeve gastrectomy, and laparoscopic omega-loop gastric bypass were 0% for EOSS 0 and 1.6% for EOSS 1. The postoperative complication rates were 8.2% for EOSS 2 and 22.4% for EOSS 3. The authors suggest that patients should have bariatric surgery when their EOSS stage is 2 or less to prevent metabolic disease and reduce the risk of complications following surgery. View article 
Improved Muscle Mitochondrial Capacity Following Gastric Bypass Surgery in Obese Subjects

Energy restriction often results in less weight loss than expected. A study was conducted to test if mitochondrial efficiency contributes to reduced energy expenditure during weight-loss maintenance. Prior to and six months following gastric bypass surgery, muscle biopsies were collected and mitochondrial efficiency was measured. Participants had a decrease of 7 kg/m2 in BMI (from 38.5 to 29.6 kg/m2). There was a significant improvement in mitochondrial efficiency (less proton leak) in the skeletal muscle samples, which results in less "wasting" of energy. These results suggest that a change in mitochondrial proton leak following weight loss may be partly responsible for the decrease in basal metabolic rate that is often observed following weight loss. Adding Phentermine to Canagliflozin Significantly Increases Weight Loss in Patients with Obesity
Hollander, P., et al. 319-LB. Presented at American Diabetes Association Scientific Sessions; June 10-14, 2016; New Orleans.  
Phentermine (15 mg) was prescribed in combination with 300 mg of canagliflozin in 334 adults without type 2 diabetes who had a BMI between 30 and 50 kg/m2. This 26-week, phase 2 study found that patients who received phentermine in addition to canagliflozin had weight loss of 7.5% compared with 4.1% for patients who received only phentermine and 1.9% for patients who received only canagliflozin. The addition of phentermine to canagliflozin caused a significant reduction in systolic blood pressure (-4.2 mmHg). The authors report that the combination was generally well tolerated, with no new or unexpected safety signals. This preliminary report suggests that this medication combination may be a useful therapy for obesity, but more research is needed for longer time periods. View article
