A recent literature review commissioned for the U.S. Preventive Services Task Force examined the benefits and harms of intensive behavioral counseling about diet and exercise on patients with risk factors for heart disease. The review found that intensive behavioral counseling can help lower these patients' risk for heart problems. On average, the counseling led to modest improvements in cholesterol, blood glucose, blood pressure, weight and risk of diabetes after one to two years. Unfortunately, this kind of individualized care is generally not paid for by the U.S health system, and the patient must pay out-of-pocket to gain the benefits of individual behavioral counseling.

In June 2013, nine thought leaders convened in an expert forum to discuss both the documented and the potential adverse events and safety issues related to the major pharmaceutical agents for diabetes treatment. The group reviewed summary data for each of five drug classes: insulin, sulfonylureas, thiazolidinediones, glucagon-like peptide-1 receptor agonists, dipeptidyl peptidase-4 inhibitors, and sodium glucose co-transporter 2 inhibitors. They outlined risks and benefits that must be weighed and discussed with patients when developing individualized treatment programs. The international panel consisted of Drs. John B. Buse, William T. Cafalu, Stefano Del Prato, Phillip D. Home, Derek LeRoith, Michael A. Nauck, Itamar Raz, Matthew C. Riddle, and Julio Rosenstock. This article summarizes well-known and theoretical safety concerns related to these drug classes, as well as the panelists' opinions regarding their best use in patients with type 2 diabetes.

Ethnic-specific obesity cutoffs for diabetes risk: Cross-sectional study of 490,288 U.K. Biobank participants

An analysis of the U.K. Biobank cohort (502,682 U.K. residents ages 40 to 69) was conducted to compare the relationship between adiposity and diabetes across different ethnic groups and then develop ethnic-specific obesity cutoffs (using body mass index [BMI] and waist circumference) for black, Chinese, and south Asian populations that equate to the specific obesity cutoffs developed for white populations in terms of diabetes prevalence. The analysis found that for the equivalent prevalence of diabetes at a BMI of 30 kilograms per meters squared (kg/m2) in white participants, BMI equated to the following: south Asians, 22.0 kg/m2; black, 26.0 kg/m2; Chinese women, 24.0 kg/m2; and Chinese men, 26.0 kg/m2. For women, a waist circumference of 88 centimeters (cm) in the white subgroup equated to the following: south Asians, 70 cm; black, 79 cm; and Chinese, 74 cm. Among men, a waist circumference of 102 cm equated to 79, 88, and 88 cm for south Asian, black, and Chinese participants, respectively. Based on these findings, obesity should be defined at lower thresholds in nonwhite populations.
