The Canadian Obesity Network (CON), creator of the 5A's of Obesity Management, has released a new toolkit that modifies the framework from the original 5A's to help primary care practitioners discuss weight gain during pregnancy with their patients. The most common genetic cause of obesity is from a mutation in the melanocortin-4 receptor (MC4R), which is associated with hyperphagia. The effect of this receptor mutation on the brain's response to food cues is not known. A recent study evaluated the role of mutations in this receptor with brain responses to food cues using fMRI scans. The researchers found that this receptor is likely involved in the brain's reward center, as it had a blunted response to food cues in individuals affected by obesity, relative to lean controls. Individuals affected by obesity with the MC4R mutation had similar activity in the reward centers of their brains as people with a normal weight when both groups were shown a picture of appetizing food. However, researchers found that the reward centers were underactive in people affected by overweight and obesity and who do not carry the mutation. Understanding this pathway may help in developing interventions to limit the over consumption of highly palatable foods that can lead to weight gain.

Children are reaching puberty much earlier than a century ago. The onset of puberty in girls was 14.6 years in 1920 compared to 10.5 in 2010. A study was conducted to analyze the relationship between sex hormone-binding globulin (SHBG) and the timing of puberty. The study found that children who were heavier at 5 years of age tended to have lower levels of SHBG. SHBG binds to androgen and estrogen, so the levels are initially high in childhood but decline significantly before puberty to allow sexual maturity to happen. Hormonal disturbances associated with weight gain and inflammation may explain the link between obesity and the declining age of puberty.

The impact of interventions to prevent obesity or improve obesity-related behaviours in children (0-5 years) from socioeconomically disadvantaged and/or indigenous families: A systematic review

Children from low socioeconomic backgrounds have higher rates of obesity than children of more affluent families. Low-cost obesity treatment interventions are challenging and often ineffective. This review article examined the effectiveness of interventions aimed at preventing or improving obesity in children up to 5 years old from socioeconomically disadvantaged families.

ASBP signs on to Sunshine Act letter
Today, a letter that ASBP, along with many medical associations and medical specialty societies, signed on to was sent to the Centers for Medicare and Medicaid Services (CMS) proposing "an expanded time frame to allow recipients to register, review and dispute their data in the Open Payments System before publication." Those who signed on to the letter support the Sunshine Act itself but would like to see further changes made to the Open Payments System. Click here to read the letter sent to CMS.
