Many people believe that breakfast is the most important meal of the day for general health. A randomized controlled trial examined the causal links between breakfast habits and components of energy balance in free-living humans. The study found that there was no metabolic adaptation to breakfast in terms of resting metabolic rate or suppression of appetite. Physical activity thermogenesis was higher with breakfast than with fasting, but body mass and adiposity did not differ between conditions at baseline or follow-up. Adipose tissue glucose uptake and systemic indices of cardiovascular health also did not differ with or without breakfast. Blood glucose may be more variable throughout the day with fasting than with breakfast.

A randomized, crossover control trial was conducted to evaluate the effect of increasing protein intake, at the expense of carbohydrates, on intrahepatic lipids (IHLs), circulating triglycerides (TGs), and body composition in healthy men and women on a high-fat, hypercaloric diet. Two diets were compared after consuming them for four weeks: high-protein (HP) (38 percent fat, 25 percent protein, 37 percent carbohydrates) or normal-protein (NP) (39 percent fat, 15 percent protein, 45 percent carbohydrates). IHL and plasma TG tended to be lower during the HP condition compared to the NP condition. Fat mass was significantly lower with the HP diet than with the NP diet, whereas fat-free mass was higher. This study suggests that a high-protein, high-fat, hypercaloric diet has positive effects on lipid metabolism and body composition.
Topiramate for weight reduction in adolescents with severe obesity

The effect of topiramate plus lifestyle modification on body mass index (BMI) change in adolescents with severe obesity was examined in a retrospective chart review. Patients had attended a pediatric weight-management program and were treated with lifestyle modification plus topiramate. The chart review showed that the average 6-month percent change in BMI was -4.9, which was statistically significant. The authors concluded that topiramate with lifestyle modification was associated with clinically meaningful BMI reduction in adolescents with severe obesity. This study did not examine how adolescents' BMI changed with lifestyle modification alone; therefore, the effect of topiramate on BMI in adolescents cannot be deduced from this study.
