United Health Foundation releases annual America's Health Rankings
According to United Health Foundation's 2012 America's Health Rankings, Americans are living longer due to several medical advances, but unhealthy behavior and preventable illness threaten quality of life. While premature, cardiovascular and cancer deaths have declined since 1990 by 18 percent, 34.6 percent and 7.6 percent, respectively, Americans are experiencing troubling levels of obesity (27.8 percent of the adult population), diabetes (9.5 percent of the adult population), high blood pressure (30.8 percent of the adult population) and sedentary behavior (26.2 percent of the adult population). America's Health Rankings is an annual comprehensive assessment of the nation's health on a state-by state basis. Achieving normoglycemia is the target of glucose-lowering therapy in people with diabetes. Patients are often initially managed with lifestyle modification, then the majority of people with type 2 diabetes will require glucose-lowering therapy. Guidelines recommend metformin monotherapy as the initial therapy of choice, but if metformin fails to maintain target glucose control, the choice of second-line therapy is challenging. This study compared clinical outcomes using common alternative regimens and found that metformin plus pioglitazone had significantly lower adjusted hazard ratios for all-cause mortality, and it appears to provide superior clinical outcomes compared with the most commonly used regimen, metformin plus sulfonylurea.
Sexual Functioning and Obesity
Obesity is associated with erectile dysfunction in both prospective and cross-sectional studies. Weight loss is associated with improved sexual functioning in both men and women; however, the relationship between female sexual functioning and obesity has not been adequately described. This review explores the literature on the relationship between obesity and sexual functioning, and emphasizes the importance of addressing sexual functioning both in general practice and in weight-loss programs.
Effect of Diet Composition and Weight Loss on REE in the POUNDS LOST Study
The Preventing Overweight Using Novel Dietary Strategies (POUNDS LOST) study, conducted at the Harvard School of Public Health and Brigham & Women's Hospital and the Pennington Biomedical Research Center, compared the effects on body weight of four energy-reduced diets that differed in their targets for macronutrients. A recent analysis from this trial found that resting energy expenditure (REE) in this large study declined with weight loss, and the decline can be accounted for by the decrease in body weight. Macronutrient composition of the diet assignment did not influence the changes in REE at six months, but there were suggestive changes relating body weight to the higher fat diets. There was an adaptive metabolic response to weight loss at six months that had disappeared by 24 months. |
