NACCHO will accept Model Practices applications Oct. 1-31, 2012. Since 2003, NACCHO's Model Practices Program has recognized and shared outstanding local public health practices through the Model Practices Database. Submit a practice and contribute to the overall improvement of public health through evidence-based methods. For more information, contact the Model Practices team at practices@naccho.org.
