See how Pathway's VIVA! Plus care partners--Physician Services, Clinical Rehabilitation Services and Medication Management Partners--are helping improve Pathway residents' lives.

On the Hunt for Independence
Since moving to Oak Hill Supportive Living in the spring, Jerry has made huge strides in his recovery from a 2011 stroke. Pathway's therapy partner, Clinical Rehab Services, began working with Jerry right away, and he gave 100 percent effort! Jerry participates in daily fitness classes, exercises on the NuStep machine and attends outings like Camp VIVA! and horseback riding. Once an avid fisherman and hunter, Jerry is finally able to return to the sport he loves and missed.

Walk On!
When Ivy, a stroke survivor, came to Victory Centre of Park Forest from California, he couldn't walk, had limited mobility and struggled with activities of daily living. That was last March, but today Ivy is able to stand tall and walk with the help of a walker! The recovery, while amazing, was not always easy. Ivy worked tirelessly with Clinical Rehab Services physical, occupational and speech therapists to improve his life dramatically.

The Healing Power of Home
When Betty from Victory Centre of Park Forest was sent to a nursing home after a hospital stay, Clinical Rehab Services therapists regularly visited to check up on her care. She wasn't progressing and said she wanted to return "home" to Victory Centre. The nursing home planned on her staying 2-3 months, but the Pathway team had other, better plans and got her home within 3 weeks! Thanks to a doctor's order, Betty is now successfully rehabbing in the comfort and convenience of her Pathway home.

Remarkable Medicine
Ina lived an isolated life. She couldn't communicate, engage with others, had severe tremors and shuffled along on her walker with her head hung low. After Pathway partner Medication Management Partners sponsored a presentation on a new drug to treat memory impairment, Ina's Victory Centre of Bartlett caregivers and Physicians Services started her on the medicine.
Within a couple of weeks, Ina was a new person! Ina engaged in lively conversation, she attended programs, joined Bartlett's Trivia Team and started getting her hair done at the beauty shop. Ina says her "magic patch" (the medicine) allowed her to "come out of her cave."
