CMS QAPI Resources - Guides to Quality

 

The National Nursing Home Quality Care Collaborative Change Package (NNHQCCCP) is a resource that CMS has provided access to nursing home providers to support QAPI implementation.  CMS does not mandate this resource for regulatory compliance. 

 

National Nursing Home Quality Care Collaborative Change Package
The National Nursing Home Quality Care Collaborative (NNHQCC) Change Package is a menu of strategies, change concepts, and specific actionable items that nursing homes can choose from to begin testing for purposes of improving residents' quality of life and care. The Change Package was originally intended for nursing homes participating in the National Nursing Home Quality Care Collaborative led by CMS and the Medicare Quality Improvement Organizations (QIOs), to improve care for the millions of nursing home residents across the country. The Change Package was developed from a series of ten site visits to nursing homes across the country, and the themes that emerged regarding how they approached quality and carried out their work. It focuses on the successful practices of high performing nursing homes.

Workshop Available
Advancing in Excellence in Long-Term Care Collaborative is presenting the workshop "Disrupt Infections:  Creating A Community Of Prevention" as part of a 30-city tour with the Dr. Bill Thomas' Age of Disruption Tour.  

Joanne Rader, RN, MN, nationally recognized as a challenger of LTC practices, will lead the interactive sessions and live demonstrations toward creative, practical solutions for highlighting infection prevention in care communities. Questions will be answered, and Dr. Bill Thomas will close the workshop with final comments. Register Today!

The Kentucky Person-Centered Care (PCC) Program for Long-Term Care is offering two days of in-depth learning for long term care facilities (certified nursing homes) interested in renewing their efforts with Person-Centered Care practices with national and state experts including Carman Bowman, Barbara Frank, Pioneer Network, the Alzheimer's Association and more.

 

Day Two of this conference offers a special educational track for family members of current residents.

In an effort to better prepare the facilities for these surveys, CMS has authorized the release of Worksheet #1 to providers. This is the same worksheet that will be handed to facility staff upon entrance to be completed.
