Prevent Dehydration
|
Maintain Correct Fluid Balance
Based on the regulation (42 CFR 483.25, F327 Hydration) the facility must provide each resident with sufficient fluid intake to maintain proper hydration and health. The intent of the regulation is to assure that the resident receives sufficient amount of fluids based on individual needs to prevent dehydration.
During the survey process, surveyors will make observations, conduct interviews and complete record reviews to determine if the facility assessed the resident to identify current hydration status, potential risk factors and developed a plan of care with specific interventions to address identified risk factors.
If deficient practice is identified under F327, non-compliance may also be identified with related process and/or structure requirements. Click here for list

MDS 3.0 Error Spot Light
3616a Warning:
Incorrect HIPPS/RUG Value: The submitted value of the HIPPS/RUG code does not match the value calculated by the QIES ASAP System.
Cause:
For items Z0100A, Z0100B, and Z0100C, and for items Z0150A and Z0150B, the QIES ASAP system recalculates the HIPPS code, the RUG version code, and the short stay indicator. A warning is issued if the recalculated values do not match the submitted values.
For items Z0200A, Z0200B, Z0250A, and Z0250B, the QIES ASAP system recalculates the submitted RUGs code and RUG version code if the State is using a CMS-supported RUG model. A warning is issued if the recalculated codes do not match the submitted codes.
Action:
Refer to the data specifications in effect for this record to identify the acceptable values for this item.

RAI Manual Clarification of A1700
Pages 2-7 and 2-13 which has the definitions for Admission and Reentry, respectively, were not updated when the coding instructions and definitions for admission and reentry were updated in item A1700. CMS will ensure that these definitions are rectified in the next release of the RAI Manual and that the statement, "and had an OBRA Admission assessment completed," are struck from the text.
The correct coding instructions for determining Admission and Reentry are found in Section A, Item 1700, and that is the instruction that providers (both nursing homes and swing bed facilities) need to follow to differentiate between an admission and a reentry.
