Informal Appeal of Survey Findings

 

Every Long Term Care provider has the right to both an informal and formal appeal process following receipt of a completed Form CMS-2567.  There are two informal appeal processes:  Informal Dispute Resolution (IDR) and Independent IDR (IIDR).

  

SERVE ON THE THE IDR PANEL

 

Kentucky requires one member of the panel to be "a person currently engaged in the prevision of Long Term Care services who has no affiliation with the provider disputing a deficiency".  Typically, a Long Term Care Administrator is on each IDR panel.  There is no payment or travel reimbursement for an Administrator serving on a panel.  Administrators will be rotated.  If a Long Term Care Administrator is interested in serving on a panel, a confidentiality statement must be signed prior to participating in the first IDR panel.

KARES Launched on May 12, 2014

The Cabinet for Health and Family Services, Office of Inspector General (OIG) has launched a new Web Portal - KARES in support of the National Background Check program.  KARES is a fingerprint-supported State and FBI background check program launched by the OIG to help long-term care and other providers research the backgrounds of new job applicants.

Making It Stick: 
Involving Everyone for Continuous Improvement

The Office of Inspector General, Division of Health Care, Training Branch is offering a free educational opportunity for Long Term Care Providers to attend this one-day training scheduled for August 27, 2014.  

 

Learn how to make Quality Assurance & Performance Improvement (QAPI) work for you. Make it a tool to drive and accelerate improvement by integrating it into daily practice. This session will share practical strategies to engage CNAs and nurses - staff closest to the resident - in thinking critically to prevent resident declines and promote residents' well-being.  Additional class details available in the Training Brochure.  

 

Register on the KY TRAIN website - search for Course ID: 1052124. Each attendee will need to have an account on the KY TRAIN website and will need to register individually in order to receive credit for attending. Online Registration is Open Until Friday, August 22, 2014.  Prior to the training, handouts will be made available electronically to registered attendees because hard copies of materials will not be handed out at the training.

Two day Training - Scheduled for August 7-8, 2014 

The training will be held at the Fern Valley Hotel & Conference Center located at 2715 Fern Valley Rd., Louisville, KY 40213.

After this date anyone using an unsupported browser will be blocked from accessing CMSNet portal and any QIES applications.

MDS 3.0 Error Spotlight 

  

1002 Warning

 

Invalid Zip File:  Zip file contained no files.

  

Potential Causes:

  • The submitted .zip file is empty, is encrypted or not compressed.
  • The submitted file is a .zipx file.
  • The submitted file contains no .xml files. 

Action:

Contact your software vendor for technical assistance.  Refer your vendor to the MDS 3.0 Data Specifications for details of file submission requirements.

 

The name of each XML file included in the .zip file must not exceed 200 characters, including the extension.

 

The name of an XML file must not include special characters.

 

Recreate and/or rename the .zip file and resubmit.
