Welcome to the Office of Inspector General, Division of Health Care's February 2014 edition of the Long Term Care Provider Newsletter.  This newsletter is a valuable vehicle to update you on current events affecting Long Term Care.

Electronic Health Record (EHR)

 

An important part of the traditional survey process for Skilled Nursing Facilities (SNFs) is review of medical records by surveyors.  SNFs are not required to implement EHR systems, however, many facilities implement EHR systems to provide immediate access to records, improve legibility of orders or notes, enable communication across provider types, and provide clinical decision support and safety mechanisms. 

   

As a part of the survey process, surveyors perform record reviews. If a facility has an EHR system, the facility must grant access to any medical records when requested by the surveyor. The facility will provide the surveyor with a tutorial on how to use its particular electronic system, designate an individual who will, when requested by the surveyor, access the system, respond to any questions or assist the surveyor as needed in accessing electronic information in a timely fashion. During entrance conference, the surveyor will request that the facility provide a terminal where the surveyors may access the records. Moreover, surveyors will establish with the facility the process they will follow in order to have unrestricted access to the medical record. Electronic access to records will not eliminate the need for a surveyor to print a paper copy or to request a paper copy of certain parts of certain records.

MDS 3.0 Error Messages 

  

Two general types of error messages can be found on your validation report.

 

  • Fatal - "record rejected"
    • Record is not accepted into ASAP system
    • Must correct record and submit 'new' record
  •  Warnings - "issue" to "information alert"
    • Record is accepted into ASAP system
    • Determine if need to address/fix something
