Health Links coordinated care projects

 

This morning the Minister of Health and Long-Term Care, Deb Matthews, announced the creation of 19 Health Links across the province to "improve co-ordination of care for high-needs patients such as seniors and people with complex conditions." 

 

Asked to provide a quote in support of the project, Executive Director Adrianna Tetley said: "Our members, which include Ontario's Community Health Centres,  community-governed Family Health Teams, Aboriginal Health Access Centres and Nurse Practitioner-led Clinics, are very enthusiastic to be Health Links early adopters and we look forward to full participation as Health Links rolls out across the province.  AOHC believes everyone should have access to coordinated, integrated and comprehensive services, starting with those with the most complex conditions. AOHC supports Health Links and sees its potential to deliver people-centred services that promote the best possible health and well-being for all."

AOHC members involved in Health Links

 

We understand that a significant number of AOHC members will be partners in the 19 early adopter projects, and that Health Links will eventually be implemented throughout the province.

 

Three community health centres are co-ordinators of community projects:

  • Le Centre de santé communautaire de Temiskaming coordinates the Temiskaming Health Link In the North East LHIN;
  •  Gateway CHC coordinates the Rural Hastings Health Link in the In the South East LHIN; and in the same LHIN,
  • Belleville and Quinte West CHC coordinates the Quinte Health Link.

LHINs are expected to make local announcements outlining partner organizations. We'll keep you informed of developments.

Working better together

  

Mark your calendars and save the dates, conference is six months away. The annual primary healthcare conference  will be held Wednesday, June 5 and Thursday, June 6 at the Sheraton Parkway North hotel in Richmond Hill, Ontario, which is on the north end of Toronto.

 

The 2013 conference will highlight innovative and creative approaches to working better together to solve today's most serious challenges and address opportunities for system-wide change to achieve health and well-being for all.

Travailler mieux, travailler ensemble

 

Marquez votre calendrier et réservez les dates : le congrès aura lieu dans six mois. Le congrès annuel sur les soins de santé primaire aura lieu le mardi 5 juin et le mercredi 6 juin à l'hôtel Sheraton Parkway North à Richmond Hill, en Ontario, à la limite nord de Toronto.

 

Le thème du congrès est : Travailler mieux, travailler ensemble. Le congrès 2013 soulignera des approches innovatrices et créatrices pour mieux travailler ensemble afin de résoudre les problèmes actuels les plus graves et de saisir les possibilités de changement à l'échelle du système pour réaliser la santé et le mieux-être pour tous.

2013 Annual general meeting

 

Falling on the evening prior to the annual conference, the AGM will be held on Tuesday, June 4, also at the Sheraton Parkway. Details in the spring.

Progress Report from 25 in 5 Network for Poverty Reduction

 

Ontario's leadership hopefuls are being warned that the province will fall short of its goal to reduce child and family poverty by 25% by 2013 unless urgent action is taken. As the Ontario Liberals choose a new leader and opposition parties eye an election, a progress report by the 25 in 5 Network is calling for immediate investments to support those who are struggling.

As political parties get ready for a potential 2013 election the NDP is seeking public input to a number of questions on how to improve health care. Some of the community meetings led by NDP Leader Andrea Horwath are being held in CHCs.

National Medicare Week

 

Tweet your support December 2-8th

 

The Canadian Health Coalition has designated the week of December 2-8, 2012 as National Medicare Week. This year's theme is the need for Federal Leadership in health care. Citizens from across Canada will be writing, calling and meeting with their MPs to build support for a new 10-year Health Accord, and call on Prime Minister Harper to return to the negotiating table with the provinces and territories.

Tips on how to access the Community Start Up and Maintenance Benefit (CSUMB) for your clients

 

The government is cutting this benefit for people on social assistance as of January 1, 2013. CSUMB provides up to $799 over a 2 year period and can be used for housing issues such as: deposits and hook-up fees for utilities; last month's rent deposit; moving costs; rent arrears to prevent eviction.
