Healthy Communities Partnership logo The Healthy Communities Partnership is dedicated to preventing heart attack, stroke, cancer, diabetes and other leading chronic disease-related causes of death and disability of residents in Dubois, Gibson, Pike, Posey, Spencer, Vanderburgh and Warrick Counties through initiatives including tobacco cessation, healthy eating, active living, preventive services and more. The Partnership recently created a video that further explains how it is attempting to make our region healthier; included on the video is a segment about the Deaconess VNA Plus "Heal at Home" program for vulnerable seniors with chronic illnesses.
