Choices Coordinated Care Solutions is hiring Care Coordinators

Indianapolis, Fort Wayne, Muncie and Bloomington

The Care Coordinator will work in full partnership with families and their chosen team members to develop a plan of care focused on keeping youth in the community. The Care Coordinator coordinates the implementation of the plan, identifies providers of services or family based resources, facilitates family team meetings and monitors all services authorized for the youth's care. In addition to this, the Care Coordinator assures services are delivered in a manner consistent with strength-based, family driven, and culturally competent values.
