Just a friendly reminder that a newly revised Proposal Routing Form (PRF) has been made available since the beginning of December of this year at:
- We encourage all Principal Investigators (PIs) to download & save PRF prior to filling it out for each proposal.
- Older version dated 6-25-15 of PRF will still be accepted until the end of January, 2016 to allow some time for the transition.
- When searching for an internal form, it is recommended to search via UMassD website rather than Googling it to make sure the most recent version is brought by the search.
- All the updates were made based on the feedback received from the faculty, administrative assistants and SPA staff and were highlighted in detail in UMD Announce e-mail sent out at the beginning of December, 2015.
- Please pay special attention to PRF Section VI: Conflict of Interest
- Sign & submit Summary Disclosure Form along with the PRF prior to the proposal submission, as instructed on both forms: PRF & the Summary Disclosure Form.
- Summary Disclosure Form is required for all PI(s), Co-PI(s)/key personnel for each project.
- Direct links to separate Summary Disclosure Forms are included in PRF and now available only 1 click away for easier access.
