Updated Forms

Check out our Forms & Templates Toolkit for the most recent and updated forms for proposal submission & grants administration.

A new revised Proposal Routing Form (PRF) is being rolled out on July 1, 2015 for all proposals going forward.  A grace period of 2 months is established during which older versions of PRF will be accepted to accommodate a smoother transition.  Starting September 1, 2015 only new revised PRF should be used for all proposals.

The new PRF brings the following benefits, since it is:

  • only 2 pages long (now 1 page shorter)
  • user friendlier fillable pdf form with sum function and drop down menus which minimize errors/typos
  • capturing only required information for data entry
  • incorporating the links to applicable policies & forms, plus, prompts/notes defining and clarifying what is required
  • eliminating formerly required separate cost share form which is now incorporated into new PRF.  If additional space for cost share is required, the current cost share form can be used (optional).
  • equipped with an electronic signature feature built-in which makes it easier to route for approvals
  • used by the Institutional Advancement Corporations and Foundations office to help streamline and unify pursuit of external funding.

Revised Summary Disclosure Form

is being rolled out on July 1, 2015.  The same grace period as for the revised PRF applies.  The Summary Disclosure Form is required for all proposals.

If you have any questions regarding the revised form please contact Andrew Karberg at akarberg@umassd.edu or ext. 9880.

Revised budget template includes the new F&A and fringe benefits rates.  Please use the updated template to assure you are budgeting at the current rates for all your future projects.

The ECRT effort reporting solution has been upgraded to its latest version, v5.1, in an effort to provide the UMass System with the most recent functionality and capabilities.

Benefits for individuals who certify their own effort and/or the effort of others include:

  • More detailed emails that list the names and dates of statements that an individual needs to certify. Emails are only sent to individuals who have outstanding statements to certify.
  • A new home page designed to immediately communicate outstanding certifications for the individual. A dynamic task list updates as certifications are completed to help answer the question "Am I done?".
  • A new certification page that allows greater flexibility during the certification process. An individual can certify up to 25 statements on a single page. They can also sort the statements that they need to certify by name, department, or project relationship.
  • A Search window displayed on every page of the system for instant search and navigation to pages within the system related to people, projects, or department information.
Sincerely,
