Action Required to Complete Reg for Fall Soccer
Thank you for registering your child(ren) for River Park Soccer.

We have processed your registration packet, however the signed membership (1601) form is missing.
Unfortunately, a faxed or scanned signature is not acceptable for this purpose.
Therefore we can offer you two choices.
1) Come by 5258 Minerva Ave, 95819 (between Moddison & Callister) during daylight hours.  Locate the plastic file box on the porch and find the envelope labeled with your name.  Sign all 1601's in the envelope, reclose the envelope and put it into the mail slot.  Do not put back into the file box.
2) For each player you registered, download a new copy of the form, fill it out completely, sign and mail to PO Box 192077, Sacramento, CA 95819.
Warning:  All corrections must be received by June 25 or the player will not be put on a team until after all on-time registrants have been assigned.

You may receive another message stating "No action required" if another player you registered had all documents complete.   Please contact the registrar if you have questions.
