The CDC-led National Diabetes Prevention Program (NDPP) is an evidence-based lifestyle change program for preventing type 2 diabetes. It can help people cut their risk of developing type 2 diabetes in half. Participants work with a lifestyle coach in a group setting to receive a 1-year lifestyle change program.

"Living Well" 
Chronic Disease Self-Management Programs
The "Living Well" program is being offered nationwide and is impacting lives by enabling people to better understand their chronic conditions while equipping them to effectively handle their care. Graduates of the program demonstrate significant improvements in exercise, symptom management, communication skills, fatigue and social activities. They spend fewer days in the hospital and tend to have fewer outpatient visits. Find a Program in your county!

March 25th is diabetes alert day
Do your employees know if they are at risk?
During this one-day "wake-up call" on Tuesday, March 25, 2014, take the Diabetes Risk Test for type 2 diabetes. The Diabetes Risk Test asks users to answer simple questions about weight, age, family history and other potential risk factors for pre-diabetes or type 2 diabetes. doctor_pills2.jpg The P3 Program TM (Patients, Pharmacists, Partnerships) brings specially-trained pharmacists into the workplace to provide care management services for patients with diabetes and hypertension. P3 pharmacists provide employees with medication therapy management, education, and lifestyle coaching while working in collaboration with the patients' physicians and other community resources. P3 medication therapy management services are designed to improve the health and quality of life of chronic disease patients while decreasing the employer's health care costs.  Additional information is available from the University of Maryland School of Pharmacy.
