QIDE REC Now Offering FREE Hypertension and Diabetes Training Sessions for Delaware Community Health Workers

Are you a Community Health Worker (CHW), which is a frontline public health worker who is a trusted member of and/or has a close understanding of the community served, in need of additional training to help people in your community better manage and prevent hypertension and diabetes?

Both of these chronic diseases are plaguing tens of thousands of Delawareans. Since CHWs play such a vital role in bridging the gap between ethnic, cultural or geographic communities and their healthcare providers (and serve in roles such as community health advisor, outreach worker, community health representative, peer counselor or lay health advisor), Quality Insights of Delaware is offering FREE trainings to CHWs to enhance their knowledge and understanding of both diseases.

Lunch & Learn: Diabetes and Hypertension Quality Improvement Opportunity

EventCalendar The prevalence of hypertension and diabetes in Delaware is now at a critical level. In a concerted effort to reduce these numbers, the Division of Public Health, Delaware Department of Health & Social Services (DHSS), and Quality Insights of Delaware - Regional Extension Center (QIDE REC) have initiated the second phase of the State's initiative to help people with high blood pressure and diabetes through the use of National Quality Forum (NQF) measures by their providers. These measurements are part of a nationwide effort to provide direction for improving the quality of health and health care in the United States.

IMPORTANT REMINDER: Submit Your Agreement to Continue Working with QIDE REC to Combat Hypertension and Diabetes              

As we move into the upcoming year of the Hypertension and Diabetes Department of Health work, Quality Insights of Delaware - Regional Extension Center (QIDE REC) is required to obtain your written agreement of participation.  If you are interested in participating in this program, you will need to sign and complete one of the following documents:
  • If you currently have an ACTIVE consulting agreement with QIDE REC, you will need to complete the Addendum to the Consulting Services Agreement and submit it to QIDE REC by end of business Monday, August 17, 2015. 
  • If you DO NOT have an active agreement with QIDE REC or are unsure if you have an active agreement, you will need to complete the Memorandum of Understanding and submit it to QIDE REC by end of business Monday, August 17, 2015.
Please submit your signed Addendum/MOU to QIDE REC via e-mail to Maureen Sandberg (preferred) or fax to 610.783.0374.

 

Your participation in this upcoming year's work creates a foundation for your practice's quality improvement efforts, as well as prepares your practice for future value-based payment models.  Additionally, it allows your practice to receive a limited scope of meaningful use services at no cost (current rate is $150/hour).  As a reminder, below is a list of the practice expectations for the project:

  • Designate an individual to act as the primary contact for QIDE REC.
  • Submit monthly or quarterly reports for NQF 0018 (hypertension) and NQF 0059 (diabetes) measures.
  • Participate in a short survey and workflow analysis regarding current hypertension and diabetes processes with the practice.
  • Participate, when possible, on project sponsored/hosted webinars.
  • Consider best practice workflow modifications for implementation within your office.
  • Share provided education materials with your staff and, as appropriate, patients.
