Retrieving Your Final Performance Score Report and/or Performance Score Certificate
Your facility's Final Performance Score Report (PSR), as well as the Performance Score Certificates (PSCs), in both English and Spanish are now available for you to download at the www.dialysisreport.org website. These items are for posting according to statutory requirement.

Additionally, beginning January 15, 2015, all ESRD QIP operations on the DialysisReports.org website will cease.  Facilities needing a replacement PSC for compliance purposes after this date may request one via email at ESRDQIP@cms.hhs.gov.  Because of this impending shutdown of the DialysisReport.org website, please download and save an electronic copy of your certificates before January 15, 2015 for your reference.

The Centers for Medicare & Medicaid Services (CMS) will make future announcements regarding the operation of the PY 2016 ESRD QIP with regard to dissemination of reporting documents and operation of the Preview Period.

As previously indicated to you, technical specifications, measure analyses and template documents previously provided on DialysisReports.org are now available on the CMS.gov website.

As always, should you have any questions  or problems retrieving your PSR or PSC, please let the Network Staff know.
