IMPROVING CARE COORDINATION AND QUALITY INITIATIVE GOALS

 

The goals of the Improving Care Coordination and Quality initiative include:

  • Reducing hospital admissions and readmission rates in the Medicare program by 20 percent by 2019
  • Increasing community tenure, as evidenced by increased number of nights spent at home, for Medicare beneficiaries by 10 percent by 2019
  • Reducing the prevalence of adverse drug events that contribute to significant patient harm, ER visits, observation stays, hospital admissions or readmissions occurring as a result of the care transitions process

Welcome to the Quality Improvement Organization Program's 11th Scope of Work (SOW), directed by the Centers for Medicare & Medicaid Services (CMS).  This SOW is driven by key initiatives to achieve CMS' three-part aim of better care, better health and lower costs

 

Improving Care Coordination and Quality Initiative

Quality Insights seeks to serve as a cornerstone of care coordination by forming innovative community coalitions that unite stakeholders with a shared vision of improving coordination of care for patients with across our region. Our goals are to reduce hospital admissions and readmissions, prevent adverse drug events, and increase the time that Medicare beneficiaries spend at home versus in an institutional setting.

 

Our efforts and communities include acute and post-acute providers across all care settings as well as long-term services and supports (LTSS) and other community partners. We are also placing special emphasis on recruiting and sustaining the involvement of Medicare beneficiaries and their families to participate in all care coordination activities and planning processes.

 

Many providers have worked with Quality Improvement Organizations (QIOs) in the past. As part of this new Scope of Work, five former QIOs have joined forces to form the Quality Insights Quality Innovation Network. These partners are:

  • Quality Insights of Pennsylvania
  • West Virginia Medical Institute

More than 10 million Medicare beneficiaries, approximately 20 percent of older Americans, are diagnosed with five or more chronic conditions. Effective care management of this population is often complicated by several other health and social risk factors. Unfortunately, multiple studies reveal that the health care needs of complex older adults are poorly managed, often with devastating human and economic consequences.

 

The Transitional Care Model (TCM), designed by Dr. Mary Naylor and a multidisciplinary team of colleagues at the University of Pennsylvania, addresses these challenges through a proven care model designed to prevent or minimize the effects of acute illness among chronically ill older adults. The TCM provides an evidence-based foundation for needed change. Findings from multiple clinical trials have consistently demonstrated the positive impact of the TCM on older adults' outcomes while reducing total costs of healthcare.

 

Registration is open for two upcoming Foundations of Transitional Care seminars:

  • January 14, 2015 - February 3, 2015
  • April 1, 2015 - April 22, 2015
