Submit your abstract for FMM 2014 
Physicians, Residents, Students, Healthcare Professionals and Non-Healthcare Professionals are invited to submit proposals for the 3rd Annual Family Medicine Midwest Conference.
Submit an abstract within any of the following teaching method categories:

* Beginner Level Workshop
* Intermediate Level Workshop
* Seminar/Lecture
* Case Study
* Panel Discussion
* Round Table Discussion
* Paper & Research Presentation
* Poster Presentation

All submissions must be submitted through the online form.  Click here for the abstract form.

**Students are encouraged to submit for their upcoming summer projects as a work in progress.  Student-run clinics are also encouraged as abstract topics. 

Notifications will be sent by email to the primary presenters by June 15th.  Poster presenters will
be notified by email by July 7th. Presenters will be responsible for ALL expenses related to the conference preparation, travel, and participation (e.g. registration fees, hotel, travel, preparation of presentation materials,
workshop supplies, etc.). Speakers are required to register for the full conference.

Scholarships
Students scholarships consist free conference registration and a stipend help with travel expenses. All students who submit abstracts should also apply for a scholarship.  The deadline to apply for scholarships is Aug. 18  

Donations are needed to fund scholarships.  The more funds we have, the more students will be there!  
Awards! Nominate someone today
All 2014 Family Medicine Midwest Awards recipients will be honored during the 2014 Family Medicine Midwest Conference October 10-12 in Minneapolis, Minn. 
Travel expenses not included, but any student award winner would be provided a standard FM Midwest scholarship.  A nomination should include a 2-page maximum letter summarizing the individual's contribution and a copy of the nominee's resume. Nominations will be received through July 31, 2014.
