From the President
It has been over a year since the last interRAI Newsletter, but it has been quite a year for interRAI.
The work of interRAI has gradually expanded into new fields from its beginnings in long term care and community aged care, to encompass mental health, disability services, hospital care and most recently paediatrics. To enable a wider group of contributors, it has become necessary to re-arrange our structure to enable a wider group of contributors.
We have created 3 major working groups (or "interRAI Networks") focussing on integrated care and aging (iNICA); acute hospital care (iNAC) and mental health (iNMH); we may form more in the future as newer applications mature or we develop new instruments. These groups meet at least yearly in person, and frequently by teleconference to promote instrument development and research in their respective fields. Every two years, interRAI as a whole will meet to be sure that information is shared across the Networks. Coalescing these groups is the interRAI central "Instrument and Systems Development" committee, which sets policy around instrument development, reviews scientific developments, and is the final arbiter of new systems. Other working groups attend to general policy and operations. interRAI membership now approaches 100 members from 35 nations.
interRAI's fundamental mission and our values remain unchanged; we continue to focus on the use of science to support decision-making in the health care field, and remain dedicated to be an independent, non-profit, voluntary organization. In April 2016, interRAI will conduct it first global "open" conference in Toronto, Canada in conjunction with its own plenary meeting. This conference will bring together all interRAI members as well as a much wider community of people who have an interest or engagement with interRAI. This meeting will be widely publicised, including in future editions of this newsletter.

New interRAI Acute Care Screener: The Purple Box Assessment
interRAI has developed a new assessment system to support screening of patients who arrive in acute care hospitals. The system is designed to screen for a multitude of syndromes and risks associated with hospitalisation such as delirium, dementia, falls, and pressure ulcer.
The assessment comprises 59 clinical observations, supported by a range of algorithms to generate a problem list, a small number of scales and diagnostic and risk screening tools. It is expected to be applied to ALL adult patients, and will be administered by nursing staff at arrival. Preliminary testing suggests that it requires 15 minutes to complete. It is envisaged as a companion instrument to the interRAI Acute Care assessment system, which is designed for more in depth assessment of older patients with complex functional and psychosocial needs.
The "Purple Box Assessment" (or PBA) arises from its place in the hospital "mini-suite" depicted below.
It is approved as an "alpha" version by interRAI's Instrument and Systems Development committee. This level of development denotes its readiness for field testing.
An extensive program of research is planned by iNEAC over the next 2 years to fully test its clinimetric properties, and to prepare a final version with associated applications and training materials. For further information, contact Len Gray.

Paediatric Home Care Manual now Available
The interRAI Pediatric Home Care Assessment (PEDS-HC) is a standardized assessment tool developed for use in programs serving children with special health care challenges. The PEDS-HC instrument is designed to be used to assess the home care challenges of children and youths ranging in age from 4 through 20 (in the USA) who are seeking or receiving long-term services or supports. Drs Charles Phillips and Catherine Hawes (interRAI Fellows) are the primary authors. This book contains introductory material, an item-by-item guide to completing the assessment form, and the assessment form itself.

Brief Mental Health Screener proves successful
The interRAI Brief Mental Health Screener (BMHS), a unique mental health assessment form developed by Ron Hoffman, PhD with the help of interRAI Board member and academic advisor Dr. John Hirdes, is becoming more widely used in Canada, following a series of successful pilot studies.
The one-page assessment form is designed to establish a "common, descriptive, but efficient language" between police officers on the street and medical staff in the hospitals. The BMHS grew out of Hoffman's observation that there is a frequent disconnect between law enforcement and medical personnel when the police bring seemingly mentally disturbed individuals into the hospital.
