Your ASLM2012 continuing education (CMLE) credits are waiting to be claimed.

 

ASLM would like to use your feedback to make improvements for ASLM2014.  Please take the time to complete the survey today!

 

Deadline for evaluation submission:  20 January 2013

Evaluation, Continuing Education, and Certificates of Attendance

In order to obtain your continuing education credits, the conference evaluation found at www.ASLM2012.org needs to be completed. The link to the evaluation is located on the right side column. Your feedback is important for ASLM2014!

Please follow the conference evaluation directions to ensure submission is received.

 

CMLE and Certificates of Attendance will only be available through successful conference evaluation completion. A successful submission is indicated by receiving a confirmation message on the screen. Submissions will be verified and the appropriate certificates will be sent to you via email within 30 days of conference survey evaluation closure.

 

 The deadline for completing the evaluation is 20 January 2013

  

Are you having trouble completing the survey?

There have been reports from some conference attendees that the survey locks up and prevents submission. If this happens, please use our alternate conference evaluation site. Alternate Survey Link
