Please complete the conference evaluation survey found at www.ASLM2012.org on the right side column, so we can incorporate your feedback into plans for ASLM2014! Please follow the conference evaluation directions to ensure submission is received.
CMLE and Certificates of Attendance can be obtained by completing the conference survey above. Submissions will be verified and the appropriate certificates will be sent to you via email within 30 days of conference survey evaluation closure.
Are you having trouble completing the survey?
There have been reports from some conference attendees that the survey locks up and prevents submission only in Internet Explorer. If this happens and are limited to Internet Explorer, please use our alternate conference evaluation site. Alternate Survey Link
