Provider Focus: Florence Crittenton's Community Based Services

Did you know that in addition to the holistic services provided in the Therapeutic Group Home (TGH) Program, Florence Crittenton provides outpatient behavioral health services to male and female youth, adults and their families to achieve a higher level of independence and functioning? Services are designed to be short-term in nature, building upon a client's strengths and providing community supports while minimizing behavioral crises and a recurrence of symptoms. Treatment frequency, modality and duration will vary based upon individual client needs and preferences. Services are provided in the community and in office settings and may address family functioning, substance use, abuse and trauma, crisis planning and life and independent living skills. Community Based Services (CBS) provides excellent care delivering the following outpatient services:

Life Skills Program - community based one-on-one services to youths and adults through the Life Skills (LS) Program. The LS Program is organized into two categories: Youth and Adult Services.

-- Youth Life Skills - Serves youth, ages 14-18, who have been diagnosed with a mental illness and are eligible for state Medicaid insurance.

-- Adult Life Skills - Serves adults, ages 18+, who have been diagnosed with a mental illness and are on state Medicaid insurance.

The LS Program focuses on building and developing positive skills in youth and adults to strengthen skills and provides opportunities for increased self-sufficiency and independence. Sessions take place in a community setting such as the client's home or neighborhood, local businesses or provider office. Community-based sessions allow the case specialist the opportunity to facilitate hands-on learning for session topics.
