How to Register with CSPMP and Access the System

Gaining access to the CSPMP is a two-step process. First, you must register with the CSPMP, which is required by law if a practitioner has a license and a DEA registration. Next, a request for access needs to be submitted.

Register: To register, practitioners should visit https://azpmp.hidinc.com/apex/f?p=102:1:616967849487282 and follow the instructions to begin or renew a registration.

Access: To request access, practitioners may go to this link: http://azpharmacy.gov/pmp/access.asp. Click on the Get Database Access link. Then click on the Medical Practitioners Link. Read the instructions on the page and click on the blue link on the page. Complete the online Access Request form. Submit the form.

Once you complete the online access request form, you will receive an e-mail for the 20-minute online training course at careermap.net. Once the training is complete, you will receive two e-mails from the tech staff at hidinc.com with your user name, the database web link, your PIN number, a temporary password, login instructions and an 866 help desk phone number.
