Beginning February 19, 2013, ten nursing facilities in Las Vegas, Boulder City, and Pahrump, began a 4-year initiative with a goal of reducing avoidable hospital admissions among their long-stay residents. Funded by the Centers for Medicare & Medicaid Services Innovation Center (CMMI), the initiative provides for HealthInsight Advanced Practice Nurses and Registered Nurses to work side by side with nursing facility staff to achieve this goal. The Utah Beacon Community Project has seen significant improvement in diabetes outcomes in outpatients in the Salt Lake Metropolitan primary care practices. Our dedicated practices have streamlined use of their electronic health records (EHRs) and workflow processes over the past two years to demonstrate improved population management for over 22,000 patients. Many providers, becoming more consistent with their care processes, find that some patients need extra support to navigate the complexities of chronic diseases. Read more...

HealtHIE Nevada, the community-based Health Information Exchange (HIE), has experienced amazing growth since its launch, now including multiple hospitals, physician offices, and diagnostic centers from across the state. The Health Information Exchange increases its value to participants and patients with each additional piece of data exchanged. What Quality Improvement Organizations (QIOs) Are Doing to Help Reduce Readmissions
A study in the January 23 issue of the Journal of the American Medical Association associates the efforts of Quality Improvement Organizations (QIOs) with a marked decrease in hospitalizations and rehospitalizations among Medicare patients. Specifically, communities in which QIOs coordinated interventions that engaged the entire community to improve care reduced hospitalizations and rehospitalizations among these patients by nearly twice as much as comparison communities. The study shows that a coordinated approach involving diverse stakeholders in a community--organized and spearheaded by a locally based QIO--is a promising strategy to address care transitions.
