  MA with Patient

"New studies support limiting daily sodium consumption to less than 1,500 milligrams, according to a new American Heart Association presidential advisory.

The advisory, published in the American Heart Association's journal Circulation, is based on a thorough review of recent laboratory, animal, observational and clinical studies that reaffirm the association's 2011 advisory that limiting sodium (salt) to less than 1,500 mg per day is linked to a decreased risk of high blood pressure and cardiovascular disease, including stroke." Improving Transitions of Care: What Can My Practice Do?

"Approximately one in five Medicare beneficiaries in the United States is readmitted to the hospital within 30 days of discharge; up to 76% of these readmissions may be preventable. A common reason for readmissions is the absence of timely follow-up appointments with primary care providers to assist patients with new diagnoses, medications, and treatments." Read more...

Using Medications to Lower Low-density Lipoprotein (LDL)
Tips from the IC3 Beacon Community

"Remember to set a target dose of your chosen low-density lipoprotein (LDL) reducing therapy. You should lower LDL at least 30% per the American Diabetes Association (ADA) for a maximum therapeutic effect. The Beacon goal is 100mg/DL for our type 2 diabetes mellitus (DM2) patients, but if they have a previous cardiovascular event, the goal will be 70mg/DL to further reduce risks. When a patient's LDL result returns, calculate the needed reduction; or at least 30% reduction." Read More...
