Please have pharmacy fax us request or bring in your list at scheduled appointments. We also use the Electronic-RX refilling system for added convenience.
MISSED OR LATE CANCELLATION APPOINTMENTS:
For the courtesy of other people who need to see me...we ask for a 24-hour notice if you are unable to keep your appointment. The office policy has changed to reflect a $50 charge for non-compliance.

I stepped into the world as a small business owner several years ago and have never looked back! Although the health care business is sometimes challenging the art of nursing and getting to be a part of 'your' world is rewarding and only deepens my affinity to this field.
Although I have practiced for 21 years (in the valley for the past 16 years); being in Tempe has been a real blessing and continues to exceed my original goals. So much of my growth has come from personal referrals and I deeply appreciate them!
My practice is dedicated to Adult needs with a national specialty in Women's issues. I also specialize in preventive cardiovascular care, bio-identical hormone therapy and thyroid management for men and women. I love to write my monthly newsletter Archived Affinity newsletters
I look forward to seeing you and your friends/family. Many insurances are accepted (either under my name or under my Medical Director, Sharonelle Simmons MD). Cash pay pricing is reasonable in cases of no insurance.
