TransforMED Announces Retirement of President and CEO

The Board of Managers for TransforMED has announced the retirement of its president and CEO, Terry McGeeney, MD, MBA.

The TransforMED Board of Managers also announced the appointment of Bruce Bagley, MD, as interim president and CEO. Bagley most recently served as the medical director for quality improvement for both the American Academy of Family Physicians and TransforMED.

TransforMED and collaborative partners begin implementation of Patient-Centered Medical Neighborhood demonstration project

About 157,000 Medicare and Medicaid beneficiaries in up to 15 health systems and provider organizations across the nation will begin participating in a Patient-Centered Medical Neighborhood (PCMN) demonstration project as part of a collaboration among TransforMED, Phytel and VHA Inc.  

 

Funded by a three-year, $20.75 million cooperative agreement awarded by the Center for Medicare and Medicaid Innovation (Innovation Center), the PCMN project is scheduled to begin implementation this month.

TransforMED convenes Patient-Centered Medical Neighborhood kickoff learning collaboratives with health care organizations in seven communities
At each kickoff learning collaborative, TransforMED works with respective communities' primary care practices to develop and implement a PCMH and PCMN training program for local practices. The program incorporates consistent learning and improvement resources that will enable practices in all markets to share resources, results and experiences that promote a rapid and rigorous pace of change.

TransforMED nominated for Mr. K Award

For the second year in a row, TransforMED has been nominated to receive the Greater Kansas City Chamber of Commerce's Mr. K Award. Named for the late entrepreneur and philanthropist Ewing M Kauffman, the Mr. K Award honors the small business that best exemplifies Kauffman's business philosophies: strong growth or sustainability, outstanding employee relations, and a passion for community service. The Kansas City Chamber of Commerce is expected to announce the Top 10 Finalists for the award in March.

TransforMED improves Change Readiness Survey tool for Patient-Centered Medical Home primary care model

TransforMED's new, streamlined Change Readiness Survey (CRS) aims to make it easier and faster to measure a medical practice team's ability to manage and sustain the changes necessary for successful transformation to a PCMH.

Developed and refined based on TransforMED's work in more than 1,500 medical practices nationwide, the CRS is one element of an end-to-end assessment process TransforMED facilitators use to clarify and achieve practice transformation goals...
